摘  要:目的:分析医院阴沟肠杆菌感染的临床分布及耐药性,以指导临床合理用药。方法:对医院近3年分离出的阴沟肠杆菌的临床分布特点及耐药情况进行总结分析。结果:2005年1月-2008年4月共分离出156株阴沟肠杆菌,菌株来源主要为痰液、尿液,分别占42.9%、23.7%。药物敏感性试验显示,阴沟肠杆菌对β内酰胺类抗生素显示较高耐药;对氨基糖苷、喹诺酮类等也有不同程度的耐药,对碳青霉烯类有很高的抗菌活性。结论:阴沟肠杆菌主要引起呼吸道、尿路感染;对三代头孢菌素耐药严重,呈多药耐药性,亚胺培南仍为治疗阴沟肠杆菌严重感染的首选用药。Objective:To investigate the clinical distribution and antimicrobial resistance of infections caused by Enterobacter cloacae in our hospital for guiding the clinical application of antibiotics reasonably. Methods: Retrospective analysis of the drug resistance situation and specimen distribution was made to the E. cloacae isolated from our hospital in the recent 3 years. Results: Totally 156 strains of E. cloacae were isolated between Jan 2005 and Apr 2008 ,the percentages of these strains isolated from sputum, and urine were 42. 9% and 23.7%. The rate of resistance to cefazolin was the highest, followed by that to amoxicillin with clavulanic and ampicillin. The rate of resistance to third - generation cepholosporins was 42. 7% - 49. 7%. Tne susceptibility test results showed, All E. cloacae isolated had a relatively high resistance tol31actamase, and carbopenem had high antibioltic activity. Conclusion:E. cloacae mainly causes infections of respiratory tract and urinary tract in clinic. E. cloacae is muhidrug resistant and severely resistant to third generation cepholosporins. Imipenem remains to be the first choice to treat the severe infection caused by E. cloacae.
