重症监护病房201例严重创伤病人TRISS分析

摘　　要：目的评估我院创伤中心重症监护病房（ＩＣＵ）严重创伤病人（损伤严重度评分ＩＳＳ≥１６）的救治结果。方法对２０１例严重创伤病人应用ＴＲＩＳＳ法，计算其生存概率（Ｐｓ），以Ｍ和Ｚ检验与严重创伤结局（ＭＴＯＳ）组进行比较评估。采用第三军医大学野战外科研究所编制的计算机创伤评分软件进行分析计算。结果２０１例中预测存活１８７例，实际存活１８０例，Ｚ＝－３．１５６６，Ｍ＝０．６１８，其中钝性伤年龄＜５５岁的１０６例预测存活１０１例，实际存活９５例；≥５５岁５１例预测存活４７例，实际存活４４例。结论本组严重创伤病人救治水平与ＭＴＯＳ组相当。但同时应考虑到：（１）ＭＴＯＳ组资料来源较分散，创伤严重度不等，而本组资料均来自ＩＣＵ，ＩＳＳ≥１６；（２）ＭＴＯＳ组＞６５岁者占７．９％，而本组占１０．６％。因此，说明本组严重创伤病人救治水平较高。Aim To evaluate the therapeutic effect on patients with severe trauma in the intensive care unit (ICU) in our hospital. Methods The probability of survival (Ps) was calculated on 201 trauma patients in ICU in our trauma center by means of the trauma and injury severity score (TRISS) method, which could considerate the physiological dysfunction, anatomic injury and the patients' age. The results were compared with the major trauma outcome study (MTOS) by M and Z test. All results were dealt statistically with the computer statistics analysis system of our university. Results Among the 201 cases, the calculated expected survivals were 187 and the actual ones 180 (Z=-3.1566, M=0.618). Among 106 patients (age <55) with blunt trauma, the expected survivals were 101 and the actual ones 95. Among 51 patients (age ≥55) with blunt trauma, the expected survivals were 47 and the actual ones 44. Conclusion There is no significant difference between our data and MTOS data in therapeutic level. On the other hand, our treating effect on severe trauma is better than that of MTOS and is worth doing more study.
