摘 要:目的观察2型糖尿病(T2DM)患者尿基质金属蛋白酶-9(MMP-9)排泄的变化,并探讨胰岛素治疗对糖尿病患者尿MMP-9排泄的影响及意义。方法以酶联免疫吸附法(ELISA)测定30例T2DM患者和28例健康成人尿MMP-9水平,糖尿病患者进行为期2~3周的胰岛素强化治疗,比较治疗前后患者尿MMP-9水平的变化。结果T2DM患者空腹血糖(FBG)、餐后2h血糖(P2hPG),糖化血红蛋白(HbA1c),尿白蛋白及尿MMP-9明显高于健康对照组(P<0.01);糖尿病患者尿MMP-9排泄率与PPG、P2hPG及尿白蛋白水平呈正相关;T2DM患者经胰岛素强化治疗2~3周后,血糖明显下降,尿MMP-9水平也明显降低(P<0.05)。结论胰岛素强化治疗可明显降低糖尿病患者尿MMP-9的排泄。Objective To observe the change of the level of urinary matrix metalloproteinase-9 (MMP-9) in the patients with type 2 diabetes mellitus (T2DM) compared with normal control group, and to investigate the effect and significance of intensive insulin therapy on the urinary MMP-9 excretion. Methods We measured the urinary MMP-9 level among 30 patients with T2DM and 28 normal subjects using enzyme linked immunosorbent assay (ELISA). All diabetic patients were given intensive insulin therapy for two or three weeks. After insulin treatment, the level of urinary MMP-9 was examined once again. Results Compared with the normal control subjects, the expression of FBG,P 2 h PG, HbAlc ,and the level of urinary albumin and urinary MMP-9 of patients with T2DM increased sig- nificantly (P 〈 0. 01 ). The level of the urinary MMP-9 of diabetic patients had a positive correlation with FPG, P 2 h PG and the levels of urinary albumin;After 2 or 3 weeks of intensive insulin therapy in type 2 diabetic patients, both of the levels of blood glucose and urinary MMP-9 had a remarkable decrease (P 〈 0. 01 ). Conclusion Intensive insulin therapy can effectively reduce the urinary MMP-9 excretion in type 2 diabetic patients.
