摘  要:目的探讨原发性或隐源性癫痫儿童的基本认知能力状况。方法采用中国科学院心理研究所研发的《基本认知能力测验》(2.0版)软件,对纳入对象进行检测。比较两组间基本认知指标的差异,并采取多元逐步线性回归分析年龄、性别、智力(IQ、受教育程度、病程、首发年龄等因素对各测试指标的影响。结果癫痫组与正常组的IQ均在正常范围且差异无统计学意义。但两组比较患儿基本认知能力测验总分值明显下降(P<0.01),具体表现在数字鉴别(P<0.01)、汉字快速比较(P<0.01)、心算(P<0.01)、汉字旋转(P<0.05)、数字工作记忆(P<0.05)及无意义图形再认(P<0.05)等方面。而双字词再认比较差异无统计学意义。推理能力是影响认知能力的最显著的因素;首发年龄则是汉字快速对比、汉字旋转最显著影响因素。结论本研究表明智力正常的原发性或隐源性癫痫儿童存在基本认知能力损害,涉及信息加工速度、心算效率、空间旋转、工作记忆、短时图形记忆等诸多认知领域。患儿的基本认知能力受推理能力影响最显著;首发年龄则是信息加工速度、空间旋转的显著疾病相关因素。Objective To explore the basis cognitive status of children with idiopathic or cryptgenic epilepsy. Method All the enrolled subjects underwent Cognitive Ability Test with the application of the basic cognitive ability test software developed by institute of psychology, Chinese Academy of Science. But it was to evaluate the basis cognitive status between two groups, and stepwise multiple regression analysis were performed to examine the relation of the scores for the basic cognitive ability with age, sex, reasoning ability, educational level, duration of epilepsy, age at onset. Results Epilepsy group and the normal group who were within the normal IQ range had significantly decreased the total score ( P 〈 0.01 ), a concrete manifestation of the digital identification ( P 〈 0. 01 ), Chinese Express compared ( P 〈 0. 01 ), mental arithmetic ( P 〈 0.01 ), Chinese characters rotation ( P 〈 0. 05 ), the number of working memory (P 〈 0. 05 ), rerecognization of nonsense figure ( P 〈 0. 05 ) and so on. Rerecognization of double-character phrases was no significant differences (P 〉 0. 05 ). Moreover, reasoning abilities was the most significant factor affecting on the scores for cognition. The age at onset was the significant factor affecting on the scores for Chinese express compared and Chinese characters rotation. Conclusion Our study suggests children with idiopathic or cryptgenic epilepsy within the normal IQ have cognitive impairment, including information processing speed, mental efficiency, space rotation, working memory, short-term graphics memory. Cognitive ability has significant impact on reasoning abilities, and the age at onset has significant impact on information processing speed and space rotation.
