摘　　要：目的建立人类T淋巴细胞白血病病毒Ⅰ（HTLV—Ⅰ）前病毒的核酸检测方法，对HTLV—Ⅰ相关高危标本进行检测，初步了解上海地区HTLV-1在相关疾病和高危人群中的流行特点，探讨核酸检测法在HTLV—Ⅰ高危人群中的检测意义。方法实验收集了69例淋巴瘤／白血病患者，72例有多次输血史的其他血液病患者以及1例HTLV—Ⅰ相关脊髓病（HAM）／热带痉挛性下肢瘫痪（TSP）疑诊患者血液标本。采用荧光定量PCR筛查HTLV-1前病毒tax基因，酶联免疫法检测血清HTLV—Ⅰ／Ⅱ抗体，对上述2种检测中出现任一阳性结果的标本进一步采用nest—PCR检测HTLV-1前病毒env基因以确定HTLV—Ⅰ的感染。结果仅HAM／TSP疑诊患者tax基因扩增出现阳性曲线；ELISA检测标本血清HTLV—Ⅰ／Ⅱ抗体均为阴性；nest—PCR扩增env基因未见特异性条带。结论上海地区小样本HTLV—Ⅰ相关高危人群中未检出HTLV—Ⅰ感染；建立的核酸检测方法能对HTLV—Ⅰ病毒感染进行有效的筛查和确认。Objective To establish PCR methods for detecting proviral HTLV-Ⅰ in subjects with HTLV-Ⅰ associated dis- eases or high risk factors for HTLV-Ⅰ infection,and to understand the prevalence of HTLV-Ⅰ in high-risk population in Shanghai. Methods Blood samples were collected from 69 subjects with lymphoma/leukemia,72 subjects with a history of receiving blood transfusion from more than three donors,and 1 subject suspected to be HAM/TSP. A re-al-time quantita- tive PCR assay was performed to screen the HTLV-Ⅰ tax gene. ELISA was followed to detect the serum HTLV-Ⅰ /Ⅱ anti- body. Nest-PCR was established to detect the proviral HTLV-Ⅰ env gene to confirm the viral infection for the positive blood samples assessed by either real-time PCR or ELISA assays. Results Proviral HTLV-Ⅰ tax gene was on-ly detected positive in the subject suspected with HAM/TSP and serum HTLV-Ⅰ /Ⅱ antibody was negative for all the sam-ples. Nest- PCR didn＇t detect env gene in the tax positive blood sample. Conclusion There is no confirmed HTLV-Ⅰ infection in this Shanghai population with high risk factors for HTLV-Ⅰ . The PCR assays on tax and env genes established in this study are effective and specific for HTLV-Ⅰ provirus screening and confirmation.
