摘  要:目的通过受试者工作特征曲线(riceiver operating characteristic curve,ROC)对Beckman-CoulterLH750血液分析仪VCS数值进行分析和临床诊断试验评价,建立最佳报警阚值。方法经显微镜检查确认为异型淋巴细胞超过20%,但仪器未有报警提示样本30例;同时收集对照样本30例,进行LH750自动上样测定及显微镜检查,评价VCS两组间差异,并通过ROC曲线计算VCS诊断价值。结果异型淋巴细胞组VCS值与正常淋巴细胞组VCS值比较,体积较大、电导升高并散射增强,差别具有显著性;体积值AUC为0.412;电导AUc为0.849,cutoff值为122,特异度为86.3%,敏感度为80.0%;光散射AUC为0.765,cutoff值为53,特异度为92.2%,敏感度为66.7%。结论VCS数值可在无白细胞分类异常情况下提示异型淋巴细胞存在。Objective To evaluate the value of VCS on hematology analyzer LH750 by receiver operating characteristic (ROC)curve and establish the best alarm threshold value of their laboratory. Methods 60 venous blood samples treated with dipotassium ethylenediamine tetraacetic acid(EDTA-K2) were collected. The white blood cell differential counts of each sample was performed with both the in strument and microscopy. Those sample were groups into two groups, the atypical lymphocyte group and the control group. The sample contained 〉20% atypical lymphocyte affirmed by microscopy but no alarm in hematology analyzer was group in atypical lymphocyte groups. ROC curve was used to evaluate the threshold values of the VCS parameters. Results For atypical lymphocyte,lymphocytes scatter,volume and conductivity showed significant differences in comparison to healthy blood donors. Areas under corresponding receiver operating characteristic curves were 0. 412,0. 765 and 0. 849 respectively for volume ,scatter and conductivity respectively. The sensitivity,specificity and accuracy of the optimal CS values were 80. 0 %, 86.3 % and 66.7 %, 92.2 % respectively. Conclusion Based on their high predictive value,research population data could be routinely used to screen for atypical lymphocyte.
