摘  要:目的总结西北地区十家教学医院2006~2007年度临床分离非发酵革兰阴性杆菌的菌群分布和耐药状况。方法常规方法培养分离医院内感染病原菌,并应用半自动或全自动细菌鉴定分析仪鉴定到种,药敏试验方法按CLSI规定的标准进行。监测数据按卫生部全国细菌耐药监测(Mohnarin)中心设计方案的要求定期上报。采用WHONET5.4软件进行数据统计分析。结果2006-2007年度西北地区10家教学医院共分离出非发酵革兰阴性杆菌3148株,其中假单胞菌属细菌1414株,占分离菌的44.92%(1414/3148),不动杆菌属细菌1403株,占分离菌的44.57%(1403/3148),嗜麦芽窄食单胞菌150株,占分离菌的4.76%(150/3148)。结论铜绿假单胞菌和鲍曼不动杆菌仍是医院内感染的主要监控病原菌,碳青霉烯类和头孢吡肟对医院内感染分离的非发酵革兰阴性杆菌仍有较好的抗菌活性,但应根据细菌的耐药表型检测和药敏结果慎重应用,同时对临床常用其它抗菌药物普遍敏感性降低。应加强对产酶菌和多重耐药菌的监测,为临床正确合理使用抗菌药物提供实验依据。Objective Summarizing 2006-2007 annual distribution of microbial population and drug resistance against non-fermented gram-negative bacillus from ten clinical teaching hospitals in northwest of China. Methods Isolates were cultured by routine methods and identified by the semi-automatic or automatic bacteria analysis system. Drug susceptibility test was assayed according to CLSI standards. Monitoring data was delivered to the Ministry of Health National Antlmiero- bial Resistant Investigation Net (Mohnarin) according to its plan and data statistic and results analysis were processed by WHONET5. 4 software pack. Results 3 148 strains non-fermentaters gram-negative bacteria were isolated from ten teaching hospitals in Northwest of China from 2006 to 2007,Pseudomonas and Acinetobacter accounted for 44.92~//00 and 44. 57%, respectively. Stenotrophmonas maltophilia was detected in 4. 76% of non-fermented gram-negative bacteria. Conclulion Pseudomonas aeruginosa and Acinetobacter baumannii are still the main pathogens,carbapenem and cefepime on hospital infection from the non-fermented gram-negative bacteria are still have good antibacterial activity. But should be based on bacterial resistance phenotype testing and careful application of drug resistance,while the other commonly used antibiotics clinical sensitivity generally lower. Should strengthen the capacity of multi-drug resistant bacteria and monitoring,for the right of reasonable use of antimicrobial agents provide experimental basis.
