摘  要:目的探讨甲状腺激素和瘦素水平对胎儿生长发育的影响。方法采用放射免疫法测定足月新生儿共70例脐血中的甲状腺激素、瘦素水平,测量新生儿出生体重、身长、记录性别及分娩方式并计算体重指数(BMI),评估新生儿营养状态。结果足月新生儿脐血瘦素质量浓度为(10.68±5.23)μg/L,性别间、不同分娩方式间差异无显著性(P>0.05);瘦素质量浓度与体重指数呈正相关(r=0.535,P<0.0l),瘦素质量浓度在大于胎龄(LGA)儿、适于胎龄(AGA)儿、小于胎龄(SGA)儿,3组间差异有显著性(P<0.01),AGA儿组明显低于LGA儿组,而高于SGA儿组。脐血瘦素质量浓度与T3、T4呈正相关,TSH呈负相关,(r=0.777,r=0.605,r=-0.76,均为P<0.01)。结论胎儿生长发育与脐血甲状腺激素及瘦素水平均有密切关系,对评估胎儿体重和生长发育均具有重要的临床意义。Objective To study the leptin and TT3,T4,TSH concentration in umbilical serum of newborns, to explore its effects on fetal development and to study their relationship.Methods Levels of umbilical venous leptin and TT3,TT4,TSH were measured in 70 newborns by radio-immunoassay(RIA)method. Neonatal bodyweight,body length,gender and mode of delivery were recorded, and body mass index was calculated to evaluate[BMI=(kg/m^2)] neonatal nutrition state.Results The full-term neonatal umbilical serum leptin level was (10.68±5.23)μg/L, there was no significant difference with gender(P〉0.05) or the mode of delivery(P〉0.05).Neonates umbilical serum leptin concent ration was positively correlated with BMI (r=-0.535,P〈0.01).There were significant differences among three groups(P〈0.01), leptin level in AGA was significantly higher than SGA group but lower than LGA group. Neonates umbilical serum leptin concentration was positively correlated withTT3(r=0.777,P〈0.01),TF4(r=0.605,P〈0.01),but negatively correlated with TSH(r= - 0.76,P〈0.01).Conclusion Leptin regulates fetal intrauterine development. It has close relationship with Tra,TT4,TSH. It has an im- portant clinical means.
