摘 要:目的观察雾化吸入重组人干扰素(r-IFN-2b)治疗婴幼儿毛细支气管炎的疗效,探讨该病的更安全方便的治疗方法。方法将113例0-2岁儿童分为三组:正常对照组、治疗对照组、干扰素治疗组,其中正常对照组为本院儿童保健体检健康儿童20名,余为2006年1月至2008年12月收住院的93例毛细支气管炎患儿,随机分为两组。治疗对照组45例,采用常规治疗并加用特布他林雾化液压缩雾化吸入;干扰素治疗组48例,在对照组治疗的基础上加用重组人α-2b干扰素压缩雾化吸入。用ELISA法分别检测血浆干扰素γ-IFN、白介素-4(IL-4),观察临床症状、体征消失时间及治疗前后γ-IFN、IL-4的水平变化。结果在临床症状、体征消失时间上,干扰素组明显优于治疗对照组(P<0.01)。毛细支气管炎患儿血浆IL-4水平在急性期即治疗前明显升高而γ-IFN水平明显降低(P<0.01)。治疗后:干扰素组血浆γ-IFN水平较治疗前明显升高而IL-4水平明显下降,差异有统计学意义(P<0.01),但较正常对照组上述指标差异无统计学意义;治疗对照组血浆γ-IFN水平较治疗前也升高,IL-4水平较治疗前也下降,但变化程度不如干扰素组明显;治疗对照组与正常组比较,IL-4水平增高而γ-IFN水平降低,差异有统计学意义(P<0.01)。结论压缩雾化吸入α-2b干扰素治疗小儿毛细支气管炎疗效好,且痛苦小、副作用少。Objective To study the effect of recombined IFNα - 2b in bronchiolitis, and find the most safe and convenient method. Methods 93 patients with bronchiolitis were divided into two groups: 45 in treat control group and 48 in IFN group, and 20 normal healthy infants were chosen as normal controls. Patients in treat control group received conventional treatment and jet atomized inhalation with terbutaline atomized liquid, while those in IFN group were treated with jet atomized inhalation with interferon α - 2b and terbutaline atomized liquid. Interferon α - 2b, IL-4 level were analyzed in serum before and after treatment using ELISA. Clinical symptoms, physical signs and length of stay were observed before and after treatment. Results Compared with normal infants, the level of IL - 4 was significantly higher ( P 〈0.01 ), while the level of γ- IFN was significantly lower ( P 〈0.01 ) in the acute episode period in infant bronchiolitis. Compared with acute episode period in IFN group, the level of IL - 4 was significantly lower ( P 〈 0.01 ), while the levels of γ- IFN were significantly higher ( P 〈 0.01 ) in the remission period. There was a little change in control group. In physical syptoms, physical signs and length of stay,patients in IFN group were better than those in treat control group significantly. Conclusion Jet atomized inhalation with interferon α -2b has significantly effect on infant bronehiolitis.
