纤支镜引导下经鼻气管插管在ICU中的应用

摘　　要：目的探讨纤支镜引导下的经鼻气管插管对ICU严重呼吸衰竭的诊治价值。方法ICU中48例危重呼吸衰竭患者,随机分为纤支镜经鼻引导气管插管组(纤支镜组)和喉镜经口引导气管插管组(喉镜组),每组24例,分别在纤支镜和喉镜引导下按常规进行气管插管术。结果纤支镜组和喉镜组一次获得插管成功率分别为100%和83.3%,插管时间分别为(3.84±2.02)min和(9.36±3.56)min。纤支镜组无患者出现并发症,喉镜组共有5例发生并发症。纤支镜组较喉镜组插管时间短,并发症少。结论在ICU中应用纤支镜经鼻引导气管插管是一种快速、安全、并发症少和成功率高的有效方法,值得临床推广应用。Objective To explore and estimate the efficacy of endotracheal intubation under fiberoptic bronchoscope through nose in severe respiratory failure of ICU. Methods 48 cases with severe respiratory failure were randomly divided into two groups, including fiberoptic bronchoscope group and laryngoscope group. 24 eases in fiberoptic bronchoscope group were intubated through nose under fiberoptic bronchoscope. The others in laryngoscope group were intubated through mouth by laryngoscope. Results The successful rates of endotracheal intubation of the two groups were 1 00% and 83.3%, respectively （P〈0.01）. The mean intubation time of the two groups were 3.84±2.02 min and 9.36±3.56 min, respectively （P〈0. 01）. Conclusions Endotracheal intubation under fiberoptic bronchoscope through nose was accurate and safe for patients of ICU.
