正常早期妊娠经阴道超声及腹部超声的显示率与HCG水平的相关分析

摘　　要：目的探讨经阴道超声及腹部超声诊断正常早期妊娠与血清HCG水平变化的关系。方法回顾2007年9月～2008年9月561例正常妊娠的孕妇早孕期在我院同时行阴道超声和腹部超声检查的声像图特征和检查当天血清HCG的结果,并进行对照分析。结果561例正常妊娠孕妇中我们检测血清HCG在7200～10800mIU/ml水平时阴道超声检查孕囊和卵黄囊的显示率均是100.0%,而腹部超声的显示率是51.9%和38.9%;血清HCG>20000mIU/ml水平时,阴道超声检查胚芽和原始心管搏动的显示率分别为100.0%和96.6%,而腹部超声的显示率是73.7%和60.2%。随着血清HCG的升高,阴道超声和腹部超声对于孕囊、卵黄囊、胚芽和胎心的显示率也升高。结论阴道超声和腹部超声的显示率和血清HCG水平变化关系密切,而阴道超声比腹部超声在诊断正常早期妊娠时更具优势。Objective To investigate the relation between diagnosis of early pregnancy by transvaginal ultrasound and abdominal ultrasound and serum HCG. Methods Look back the imaging features and the results of 561 cases of women of early pregnancy who took both the transvaginal ultrasound examination and abdominal ultrasound examination in our hospital at the same time from September 2007 to September 2008. Results When the serum HCG of 561 cases of women of early pregnancy was from 7200mIU/ml to 10800mIU/ml,it showed the yolk and pregnancy sac. The rate was both 100.0% by transvaginal ultrasound. And the rate was 51.9% and 38. 9% by abdominal ultrasound. When the serum HCG was more than 20000mIU/ml,it showed the original germ tube and heart throb. The rate was 100. 0% and 96.6% by vaginal ultrasound. The rate was 73.7% and 60. 2% by abdominal ultrasound. With the rise of serum HCG,the rate of showing pregnancy sac,yolk sac embryo and feta heart rate by transvaginal ultrasound and abdominal ultrasound also increase. Conclusion Transvaginal ultrasound and abdominal ultrasound show that the rate change of diagnosis and serum HCG are closely related, and Transvaginal ulstrasound has more advantages in diagnosis than abdominal ultrasound.
