电针结合行为疗法对早期儿童注意缺陷多动障碍临床干预的随机对照研究

摘　　要：目的：研究电针结合行为疗法对早期儿童注意缺陷多动障碍（attention deficit hyperactivity disorder,ADHD）治疗的效果。方法：4-6岁的ADHD患儿180例,其中以注意缺陷为主者60例,多动-冲动为主者60例,混合型60例。在每一类型内再随机分为试验组（电针结合行为疗法）和对照组（行为疗法）。治疗前后按DSM-IV标准对18项核心症状的变化进行观察和记录,并随访半年观察复发率。结果：（1）根据全国中医理论整理委员会多动症专业委员会制定的标准,在以注意缺陷为主的患者中,试验组显效11例,有效12例,对照组显效5例,有效12例;在以多动-冲动为主的患者中,试验组显效10例,有效17例,对照组显效4例,有效14例;在混合型的患者中,试验组显效12例,有效12例,对照组显效5例,有效12例。总体比较,试验组临床显效率及总有效率明显好于对照组,差异有统计学意义（P〈0.05）。（2）治疗后两组病例随访半年后的复发率,试验组明显低于对照组,差异有统计学意义（P〈0.05）。结论：对于早期ADHD的治疗,电针结合行为治疗能显著的提高疗效,改善核心症状,降低复发率,值得临床推广。Objective ：To study the effect of electro- acupuncture with behavior therapy on preschool children with attention deficit hyperactivity disorder （ADHD）. Methods ：180 children were enrolled by the ADHD criteria of DSM - IV and were further classified into 3 subtypes ： predominantly inattentive type （ PI, 60 eases）, predominantly hyperactive - impulsive type （ HI ,60cases） and combined type （ CT,60 cases）. In each subtype,the patients were divided randomly into experimental group,which received electro -acupuncture and behavior therapy;and control group, which received behavior therapy only. The symptom changes of ADHD were recorded and analyzed based on the criteria of DSM - IV, and the relapse rates of two groups were compared after a half - year follow - up. Results ： （ 1 ） According to the criteria of the national traditional Chinese medicine committee ,in PI subtype, 11 cases showed effectiveness with treatment and 12 cases showed improvement with treatment among experimental group ,while 5 cases showed effectiveness with treatment and 12 cases showed improvement with treatment among control group; in HI subtype, 10 cases showed effectiveness and 17 cases showed improvement among experimental group,while 4 cases showed effectiveness and 14 cases showed improvement among control group;in CI subtype, 12 cases showed effectiveness and 12 cases showed improvement among experimental group, while 5 cases showed effectiveness and 12 cases showed improvement among control group; The overall comparison manifested that the efficacy of experimental group was better than that of control group （P 〈0.05）. （2） Half - year follow - up showed the relapse rate of experimental group was significantly lower than that of control group （ P 〈 0.05 ）. Conclusion：The electro -acupuncture combining with behavior therapy has efficacy to the preschool children with ADHD for improving primary symptoms and reducing the relapse rate. It is recommendable for this combined therapy.
