清热解毒利湿法联合小剂量来氟米特治疗活动性类风湿关节炎的临床研究

摘　　要：目的:观察清热解毒利湿法联合小剂量来氟米特对活动性类风湿关节炎的疗效及不良反应。方法:活动期类风湿关节炎病人73例随机分成3组。治疗组口服清痹汤加减,炎琥宁320mg加入生理盐水250mL静脉点滴/d,共14天。口服来氟米特,前3天30mg/d,顿服,3天后10mg/d。对照1组:不口服来氟米特,加甲氨蝶呤(MTX)7.5mg周/,以每周2.5mg递增,最终剂量为20mg周/。对照2组:不口服来氟米特外,其余治疗方法同治疗组。治疗2个月,随访3个月,观察治疗结果及不良反应。结果:治疗组总有效率与其余两组比较有显著性差异(P<0.05)。治疗组症状、体征及红细胞沉降率(ESR)、C-反应蛋白(CRP)与对照组比较显著降低(P<0.05),治疗组未发现不良反应,对照2组发生1例不良反应。结论:清热解毒利湿法联合小剂量来氟米特对活动性类风湿关节炎疗效显著无不良反应。Objective： To observe curative effects and side-effects of clearing heat-toxin and eliminating dampness method combined with minute doses of letlunomide against rheumatoid arthritis. Methods ：73 patients of active stage rheumatoid arthritis were randomly divided into 3 groups. The treatment group took Qingbi decoction addition and subtraction, put on a drip intravenously yanhuning injection 320mg with physiological saline 250ml/date for 14days and took letlunomide,the first 3 days 30mg/date ,after that 10mg/date. The first control group：The patients did not take leflunomide,but took MTX 7.5mg/week instead, Increasing progressively by each week 2.5mg,the final dosage is 20mg/wee. The second control group： Except no taking leflunomide,the other treatments were the same with the treatment group. Treatments were for 2 months,making a follow-up visit for 3 months to observe treatment result and side-effects. Resu/ts ：Total effectiveness of The treatment group had significant differences compared with other two control groups （ P 〈 0.05 ）. The symptoms and objective signs of treatment group got better. Also the red blood cell rate of setting （ESR.） and the C-response protein （CRY） were reduced obviously compared with control groups（ P 〈 0.05）. There were no adverse reactions in treatment group while one in the second control group. Conclusion：clearing heat-toxin and eliminating dampness method combined with minute doses of leflunomide against rheumatoid arthritis is attached to remarkable effects and no adverse reactions.
