摘 要:目的探讨右心室流出道早搏高发生率的原因。方法应用12导联同步动态心电图检测918例室性早搏患者,计算各起源部位室性早搏发生率。结果右心室各部位早搏总发生率66.4%,左心室各部位早搏总发生率33.6%,差异具有显著性(P<0.01);右心室流出道早搏发生率为42.0%,明显高于其它各部位早搏(P<0.01)。结论右心室早搏发生率高于左心室,右心室流出道早搏发生率最高,与心肌细胞电生理异质性差异相一致;右心室流出道早搏高发生率应与右心室流出道心肌细胞电生理异质性高有关。Objective To investigate the reason of the high incidence rate of ventricular pre- mature beat from right ventricular outflow tract. Methods To examine 918 patients with ventricular premature beats by synchronism 12 - leads monitoring electrocardiogram and calculate the incidence rates of ventricular premature beat from each original position. Results The incidence rate of ventricu- lar premature beat from right ventricle was 66. 4%. The incidence rate of ventricular premature beat from left ventricle was 33.6%. The distinction was significant ( P 〈 0. 61 ). The incidence rate of ven- tricular premature beat from right ventricular outflow tract was 42. 0%, was significant higher than that from the other ture beat from premature beat original positions (P 〈 0. 01 ) . Conclusion The incidence rate of ventricular prema- right ventricle was higher than that from left ventricle. The incidence rate of ventricular from right ventricular outflow tract was highest. That was concordance with the electro- physiological heterogeneity of myocardial cell. The reason of the high incidence rate of ventricular pre- mature beat from right ventricular outflow tract was the high electrophysiological heterogeneity of myocardial cell of right ventricular outflow tract.
