摘  要:目的:鉴于应激反应与器官损伤的重要关系,文章通过观察三硝基甲苯(TNT)爆炸致伤后羊肝组织超微结构的病理变化特征,为爆炸伤的临床救治提供依据。方法:将25只羊布放在离爆心5~30 m的距离上用电启动引爆TNT致伤,伤后取肝组织用30 mg/L戊二醛固定,进行常规透射电镜标本制备,观察其超微结构病理改变。结果:电镜下肝组织超微结构的病变表现为肝血窦内淤血明显,枯否细胞增生,可见中性粒细胞、淋巴细胞浸润。肝细胞溶酶体增生,髓鞘样膜结构形成,线粒体肿胀、畸形、嵴模糊不清,内质网扩张。在5 m处存活率为0,10、15和20m处分别为40%、60%和80%,25m、30m处动物存活率为100%。结论:爆炸伤后动物存在应激性肝细胞损伤。动物存活率随动物距爆心距离增加而上升。Objective: To observe the uhrastructural pathologlcat characteristics of goat liver tissues affter blast injury from TNT explosion in order to provide an experimental basis for the clinical treatment of blast injury. Methods: Twenty-five goats received blast injury from TNT explosion 5 -30 m from the explosion center. Then their liver tissues were fixed with 3 mg/L glutaraldehyde, and the specimens prepared by routine transmission electron microscopy and observed for ultrastructural pathological changes. Results: We observed obvious congestion in the hepatic sinusoids, proliferation of Kupffer cells and infiltration of neutrophilie granulocytes and lymphocytes. We also found proliferation of lysosomes,formation of myelin-like membrane, swelling and abnormality as well as cisternal mistiness of mitochondria, dilation of endoplasmic reticulum in the hepatocytes. The survival rates were 0% at 5 m, 40% at 10 m, 60% at 15 m, 80% at 20 m and 100% at 25 or 30 m, respectively. Conclusion: Stress-induced damage to hepatocytes occurred in animals with explosive blast injury. The survival rates increased with the increasing of the distance from the explosion center.
