胆囊结石病人胆囊颈和胆囊管的组织学和组织化学研究

摘　　要：目的：了解胆囊结石病人胆囊颈和胆囊管的组织结构、粘液性质和分泌状态。方法：胆囊颈和胆囊管标本取材于15例胆囊结石病人和5例正常人胆囊。取材后用10％中性福尔马林固定，常规石蜡包埋。每个蜡块连续切片5张，分别作HE、PAS／AB2．5、醛品红／AB2．5、Masson及Weigert染色，光镜观察。结果：正常人胆囊预和胆囊管壁内存在腺体和平滑肌。胆囊结石病人胆囊颈和胆囊管壁增厚，管腔变窄，粘膜上皮出现粘液分泌改变；壁内粘液腺体增生，粘液（包括中性、硫酸和羧酸粘液）细胞数目增多，尤以硫酸粘液细胞增多更明显；平滑肌纤维有退化变性，胶原纤维数量增多，弹力纤维数量减少。结论：胆囊结石病人胆囊颈和胆囊管存在较明显的组织学和组织化学改变，这些改变可能会对胆囊的排空，结石的形成和排出产生一定影响。Objective:To disclose the histological structure, mucus eharacter and secretory status of gallbladder neck and cyshc duct in galfoladder stone patients. Method: The gallbladder neck and cystie duct specimens were obtained bo 15 gallhladder stone patients as well as 5 nonnal hurnan gallbladders. All of these specimens were fixed in 10% neutral formalin and then embedded in paraffin.5 serial sections trere cut from each paraffin block and then stained with HE, PAS/AB2.5, Aldehyde/AB2. 5. Masson and Weigert stain. Result: There were glands and smooth muscles in normal galthladder neck and cystic ducts. Wherm in gallbladder stone patient's stwimens, the epithelia of mucosa exhibited heavy mucus secretive changes and the walls were thickened. Within the wall, the mueous glands were hrperplastic and the mucous cells (incluing neutral suifated and carboxy mucous cells), espcially tLhe sulfated mucous cells, increased in number. The smooth muscles showed degneration.The qUantities of collagenous fibers incnd while those of elastic fibers decreased. Conclusion:The gallbladder neck and cystic duct in gallbladder stone patient register evident histological and histochendcal charateristic changes. These changes might have some effects on the emptying of gallbladder, stone formation and its discharge.
