壳聚糖宫颈抗菌膜联合宫颈环切术治疗宫颈病变1000例疗效观察

摘　　要：目的观察宫颈环切术后应用壳聚糖宫颈抗菌膜(chitosa),减少创面感染、促使创面愈合及预防术后出血的临床效果,以寻找预防宫颈环切术后并发症的有效方法。方法选择因宫颈病变需作宫颈环切术(LEEP)的病人1000例,随机分成两组,LEEP术后采用壳聚糖宫颈抗菌膜创面上药者500例作为治疗组;LEEP术后自然修复500例作为对照组。对照组发生出血时用云南白药粉剂局部压迫止血。结果治疗组阴道出血、流液及持续时间少于对照组(P<0.05),止血效果优于传统的方法,创面愈合时间较对照组缩短(P<0.05)。结论壳聚糖宫颈抗菌膜具有局部抗菌、止血及促进术后创面愈合的作用。使愈合时间缩短,减少创面感染、渗出、出血,减少宫颈环形电切术后并发症。[Objectives] To observe the post-operative clinical impact on loop electrical excision procedure （LEEP）, recombinant chitosa to improve the healing of the cut, prevent the post-operative bleeding, and find out effective methods of preventing LEEP complications. [Methods] 1000 patients with cervical diseases underwent LEEP in clinic. Divide them into two groups randomly, 500 patients were adopted chitosa post- LEEP as the cure group, while 500 patients naturally restored from LEEP as the contrast group, with Yunnan baiyao stopping local bleeding when necessary. [Results] Vagina bleeding timing of cure group is shorter than contrasi group （P 〈0.01）, and less recovery timing also （P 〈0.01）. The hemostatic effect is superior to traditional methods （P 〈0.05）. [Conclusion] The combination of chitsa will improve the restoration of LEEP cut, shorten its healing timing, less bleeding, less post-operative infection and less LEEP complication as well.
