低出生体重与孕期血红蛋白浓度的关系

摘　　要：目的分析低出生体重(LBW)与孕期不同阶段血红蛋白(Hb)浓度之间的关系。方法比较江苏省和浙江省4个县市1995～2000年102 489例孕妇孕期各阶段贫血和非贫血LBW儿的发生率,以及不同Hb浓度LBW儿的发生率;用Logistic回归控制年龄、文化程度、孕次、妊娠高血压疾病的影响,分析孕期各阶段Hb与LBW的关联程度。结果孕早期贫血LBW儿发生率高于非贫血,孕晚期贫血LBW儿发生率低于非贫血,差异有统计学意义(P<0.001);孕早期Hb浓度与LBW的风险呈线性趋势,Hb浓度降低,LBW的风险增加;孕中期低Hb浓度的孕妇LBW儿的发生率较高,高Hb浓度孕妇LBW儿的风险没有明显增加或降低趋势;孕晚期Hb浓度与LBW的风险呈"U"形趋势,高Hb和低Hb浓度都与LBW的风险增加有关。结论孕期Hb浓度与LBW的关系随孕期的不同阶段而不同;孕早期贫血、孕晚期Hb浓度过高或过低均与LBW的风险增加有关。Objective To examine the association between maternal hemoglobin （Hb） concentration during pregnancy and the risk of low birth weight （LBW） in a Chinese population. Methods Subjects were women who delivered in four cities or counties in Jiangsu Province and Zhejiang Province, China, during the period of 1995 -2000. Incidence of LBW was calculated and compared among groups of women with different Hb concentrations in three different trimesters. Multiple logistic regression analysis was used to address the relationship between Hb concentration and the risk of LBW while controlling for potential confounding factors. Results Anemia in the first trimester was associated with an increased risk of LBW, and a linear trend was found between first -trimester Hb concentration and the risk of LBW; whereas, a U - shaped relationship was observed between third - trimester Hb concentration and the risk of LBW： both severe anemia and high Hb concentration in the third trimester were associated with an elevated risk of LBW. The results for the second trimester presented as a transitional state between the first and the third trimester. Conclusion The association between Hb concentration and the risk of LBW differs by trimesters： while anemia or low Hb concentration in the first trimester increases the risk of LBW, both severe anemia and high Hb concentration in the third trimester increase the risk of LBW.
