摘  要:目的应用光学相干断层扫描(Optical coherence tomography,OCT)观察2型糖尿病(2DM)患者视盘周围视网膜神经纤维层(Retinal nerve fiber layer,RNFL)厚度。方法正常对照组25例36眼;2型糖尿病组40例58眼,分无糖尿病视网膜病变(Non-diabetic retinopathy,NDR),非增殖期糖尿病视网膜病变(Non-proliferative diabetic retinopathy,NPDR),增殖期糖尿病视网膜病变(PDR)3组。采用OCT分别测量正常组和糖尿病组患者视盘周围视网膜神经纤维层厚度,了解其差异性。结果正常人对照组与NDR组差异无统计学意义(P>0.05);NPDR组和PDR组与正常对照组差异有显著统计学意义(P<0.05),其中NPDR组RNFL厚度较正常对照组RNFL厚度薄,PDR组较正常对照组RNFL厚。结论OCT能定量观察糖尿患者视盘周围视网膜神经纤维层厚度的变化,为指导治疗、随访提供可靠的检测手段。Objective To investigate and assess retinal nerve fiber layer (RNFL) thickness changes of optical nerve using stratus optical coherence tomography (OCT) in patients with type 2 diabetes mellitus. Methods In a prospective study, OCT was per-formed in 58 eyes of 37 diabetic patients and 36 normal eyes of 40 control subjects. The diabetic patients were divided into three groups: non-diabetic retinopathy (NDR), non-proliferative diabetic retinopathy (NPDR), and proliferative diabetic retinopathy(PDR) groups. The difference in retinal nerve fiber layer thickness of optical nerve by stratus optical coherence tomography (OCT) between diabetic patients and normal subjects were analysed. Results There was no statistically significant difference between control group and NDR group ( P 〉 0.05 ) ,but there were statistically significant differences between normal group, NPDR group, and PDR group (P 〈0.05). The RNFL was much thicker in the PDR group, whereas it was much thinner in NPDR group. Conclusion OCT is a useful tool to observe retinal nerve fiber layer thickness of optical nerve in type 2 diabetic eyes. OCT facilitates the diagnosis, treatments and prognosis of diabetic patients.
