肾移植对男性勃起功能及性激素水平的影响

摘　　要：目的:探讨肾移植对男性勃起功能及性激素水平的影响。方法:对24例接受肾移植的已婚男性患者进行勃起功能国际问卷调查(IIEF)和手术前后性激素水平检测,调查肾移植前后不同阶段ED患病率;观察不同年龄、尿毒症病史和透析时间的患者在移植前后IIEF评分的变化;比较肾移植前后的性激素变化。结果:尿毒症期ED患病率高达87.5%,其中重度ED患病率达37.5%。肾移植术后ED患病率降低到45.9%,尤以病史短于3年、透析时间小于3个月和年龄大于46岁者勃起功能改善明显(P<0.05);肾移植后能明显降低男性患者泌乳素和雌二醇,升高睾酮。结论:肾移植能改善勃起功能,尤以病史和透析时间短的患者为明显;性激素水平的变化可能在其中发挥一定作用。Objective：To investigate the effect of renal transplant on erectile function and sexual hormones in male patients. Methods： 24 married male patients who have undergone kidney transplantation took the International Index of Erectile Function （IIEF） and the detection of sex hormone levels before and after surgery. The prevalence of ED in the different stages before and after renal transplantation was investigated. The effect the of renal transplantation on IIEF score of the patients with different history, duration of dialysis and age was observed. The lev els of sexual hormones were determined before and after transplantation respectively. Results： The prevalence of ED was 87.5% （the prevalence of severe ED 37. 5% ） and 45.9% at uremia stage and post-transplantation, respectively. The renal transplantation can significantly improve erectile function in these patients whose history less than 3 years or dialysis less than 3 months or ages older than 46 years. While the levels of prolaetin （PRL） and 18- estrodiol （E2） were decreased, the level of testosterone （T） was increased after renal transplantation. Conclusions：Successful renal transplantation can significantly recover erectile function of penis, especially for the patients with shorter history and dialysis time. The change of levels of sexual hormones partially contributes to the improvement of erectile function.
