摘 要:目的了解一氧化氮(NO)在慢传输型便秘中的作用及中药治疗的疗效。方法40只健康Wistar大鼠随机分成便秘组24只和对照组16只,分别饲喂含复方苯乙哌啶的混悬液和普通水,每5天记录一次大鼠大便粒数、大便干重及大鼠体重。饲养90d后停药1周测定肠道传输功能,再进行2周的中药方剂(黄芪、白术、枳壳等)灌胃治疗。检测治疗前后结肠黏膜NO含量、神经元型NO合酶(nNOS)mRNA表达和内皮型NO合酶(eNOS)阳性细胞分布情况。结果便秘组日均粪便粒数小于对照组,平均每粒粪便质量大于对照组,差异有统计学意义(P<0.001);便秘组首粒黑便排出时间为(420.3±129.5)min,慢于对照组的(337.2±74.7)min,差异有统计学意义(P<0.05)。近端、中段、远端结肠黏膜nNOS mRNA表达水平和远端eNOS阳性细胞分布在各组间的差异无统计学意义,近、中段结肠黏膜NO含量在对照组与便秘组间的差异无统计学意义,但便秘组远端结肠黏膜NO含量经中药治疗后下降明显,差异有统计学意义(P<0.05)。结论慢传输型便秘大鼠结肠黏膜NO及其合酶的含量变化不大,以黄芪、白术、枳壳等组成的方剂能降低便秘大鼠远端结肠黏膜NO的合成,其治疗便秘的作用尚有待于验证。Objective This study was designed to evaluate the roles of nitric oxide in the colonic mucosa of Wistar rats with slow-transit constipation and the efficacy of Traditional Chinese Medicine in the treatment of constipation. Methods Forty healthy Wistar rats were randomly divided into constipated group (n = 24) and control group (n = 16). The rats in constipated group received daily diphenoxylate 8 mg/( kg·d) to develop the slow-transit constipated model, while the control rats were fed with water without any drug. The fecal number, fecal weight and body weight of the rats were recorded every 5 days for 90 days. Intestinal transit functions were examined by activated charcoal suspension pushing test one week after stopping diphenoxylate. Constipated rats were fed with a concoction of Chinese herb such as Astraglus Base, Atractylodes Alba and Auranti Fructns daily by gastric tube for two weeks. The level of nitric oxide (NO) and the expression of neuronal nitric oxide synthase (nNOS) mRNA in the colonic mucosa were measured by nitrate reductase methods and RT-PCR, respectively, and the distribution of endothelial nitric oxide synthase (eNOS) -positive cells in the colonic wall were examined by immunohistochemical methods before and after treatment. Results In comparison with the controls, daily fecal number was decreased, and the mean weight of every pellet was bigger in constipated group (P 〈 0.001 ). The time of discharge of the first black feces was significantly later (420.3 ± 129.5 min) in constipated group than that in the control (337.2 ± 74.7 min) (P 〈 0.05). The expression of nNOS mRNA in the proximal, middle and distal colonic mueosa and the distribution of eNOS-positive cells in the distal colonic mucosa were not significantly different between constipated group and the controls regardless of treatment with Chinese herb.
