摘  要:目的探索血清铁蛋白(SF)、氧化低密度脂蛋白(ox-LDL)与2型糖尿病(T2DM)早期血管内皮舒张功能的关系。方法选择50例无大血管并发症的T2DM患者(DM组),34例年龄、性别相匹配健康个体作为对照(NC)组。高分辨血管外超声法检测肱动脉血流介导的内皮依赖性血管舒张功能(FMD)和硝酸甘油介导的内皮非依赖性血管舒张功能(NMD),检测ox-LDL、SF。结果与NC组比较,DM组SF、ox-LDL明显升高(P<0.01),而FMD、NMD明显下降(P<0.01)。分别以FMD、NMD为因变量进行多元线性回归分析,提示SF、ox-LDL与FMD明显相关(P<0.05),而与NMD无明显相关(P>0.05)。结论 SF、ox-LDL在2型糖尿病早期可能参与了血管内皮依赖性舒张功能受损。Objective To investigate the relationship of serum ferritin(SF) and oxidized low density lipoprotein(ox-LDL) with endothelium-dependent flow-mediated vasodilation(FMD)and endothelium-independent nitroglycerin-mediated vasodilation (NMD) of brachial artery in the early stage of type 2 diabetes mellitus. Methods Fifty type 2 diabetic patients without large vascular complication of DM and 34 healthy individuals(NC) were selected randomly. Under e-Tracking, high resolution ultrasound was used to measure inner diameter of brachial artery at rest. ox-LDL was measured by ELISA. SF was detected by chemilumineseent immunoassay. Results In DM versus NC group, FMD and NMD were significately lower [(4.11±1.13 vs 7.10±2.23)% P〈0.01] and [(13. 02±4. 25 vs 20.22±4.68)% P〈0.01] respectively. However, SF and Ox-LDL were significately higher [(279.19±126.68 vs 151.21±84.37) ng/ml, for SF P〈0. 011 and [(24.95 ±7. 20 vs 10.62±8. 46) ug/dl, for oX-LDL-C P〈0. 01]. Multivariable regression analysis of FMD level as a dependent variable indicated that FMD significantly related with serum SF,ox-LDL level(P〈0. 05), however,NMD did not(P〉0.05). Conclusions SF and Ox-LDL promote FMD impairment in the early stage of type 2 diabetes mellitus.
