CP患者前列腺按摩液中细胞因子及相关酶测定的临床意义

摘　　要：目的:探讨慢性前列腺炎(CP)患者前列腺液(EPS)中炎性细胞因子及相关酶学指标的变化及其临床意义。方法:将32例CP患者按美国国立卫生研究院(NIH)分类法分为三组:即慢性细菌性前列腺炎(CBP)组,慢性非细菌性前列腺炎/慢性炎性骨盆疼痛综合征(CPPSⅢA)组和慢性非细菌性前列腺炎/慢性炎性骨盆疼痛综合征(CPPSⅢB)组。EPSIL-1β、IL-2、IL-10水平均采用放射免疫分析(RIA)测定;LDH5/LDH1测定采用醋酸纤维素薄膜电泳法。结果:本文三组患者测定结果显示,CBP组EPSIL-1β、IL-2及LDH5/LDH1三项测定数值与对照组比较升高均非常显著(P均<0.01);IL-10水平与对照组比较降低非常显著(P<0.01);CPPSⅢA组IL-1β、IL-2及LDH5/LDH1水平也显著高于对照组(P均<0.05);IL-10水平与对照组比较降低非常显著(P<0.01);CPPSⅢB组则IL-1β、IL-2及LDH5/LDH1三项测定指标与对照组比较均无统计学差异(P均>0.05),IL-10水平与对照组比较降低仍显著(P<0.05)。结论:CP患者EPS中炎性细胞因子及相关酶学指标均发生了明显的变化,其测定对于CP的诊断及两种前列腺炎的分型有一定帮助。Abstract Objective To investigate the clinical significance of the changes of expressed prostatic secretion IL -1β, IL - 2, IL - 10 and LDH5/LDH1 levels in patients with chronic prostatitis. Methods Expressed prostatic secretion IL - 1β, IL - 2, IL - 10 （with Radioimmunoassay） and LDH5/LDH1 （with cellulose acetate membrane electrophoresis） levels were determined in 32 patients with chronic prostatitis and 35 controls. Those 32 patients were of 3 groups ：①chronic bacterial prostatitis （ CBP, n = 10） ② chronic pelvic pain syndrome ⅢA （ CPPS ⅢA n = 9） ③CPPS ⅢB n = 13. Results Expressed prostatic secretion levels of IL - 16, IL - 2 and LDH5/LDH1 were significantly higher in patients with chronic bacterial prostatitis （CBP） groups than those in controls （all P 〈0.01 ）, while the expressed prostatic secretion levels of IL -10 were significantly lower in patients with chronic bacterial prostatitis than those in controls（ P 〈 0.01 ）. The expressed prostatic secretion levels of Ⅱ-lβ, IL-2 and LDHS/LDH1 were also significantly higher in patients with chronic nonbacterial prostafitis chronic pelvic pain syndrome （ CPPS ⅢA） groups than those in controls （ all P 〈 0.05 ）, the expressed prostatic secretion levels of IL - 10 were also significantly lower in patients with chronic nonbacterial prostati- tis chronic pelvic pain syndrome （CPPS ⅢA） groups than those in controls （all P 〈 0.01 ）. The expressed prostatic secretion levels of IL- 1β, IL- 2 and LDHS/LDH1 were not significantly different in patients with chronic nonbacterial prostatitis chronic pelvic pain syndrome （CPPS Ⅲ B） groups from those in controls （all P 〉 0.05 ）. But the expressed prostatic secretion levels of IL - 10 were still significantly lower in patients with chronic nonbacterial prostatitis, chronic pelvic pain syndrome （ CPPS ⅢB） groups than those in controis （ all p 〈 0.05）.
