摘 要:【目的】观察手术中不同的内脏牵拉强度及持续时间与牵拉应激强度的关系。【方法】42只Wistar大鼠随机分为7组:A组牵拉10g砝码,1min。B组牵拉10g砝码,2min。C组牵拉20g,砝码1min。D组牵拉20g砝码,2min。E组牵拉30g砝码,1min。F组牵拉30g砝码,2min。G组为未牵拉对照组只完成麻醉与手术操作。观察大鼠牵拉前后呼吸循环的变化。【结果】牵拉时MAP的变化差值D、E、F组与A、B、C、G组间比较差异有显著性;A、B、C组与G组比较差异有显著性。牵拉前与牵拉时RR的差值在牵拉2min组(B、D、F组)与对照组G组间的比较差异有显著性,牵拉1min组(A、C、E组)与对照组G组间比较差异无显著性。【结论】①牵拉肠系膜的空肠动脉可复制临床牵拉动作,建立内脏牵拉应激强度模型;②增加牵拉强度和延长牵拉时间均可加重MAP的波动幅度,但MAP的变化与牵拉强度及牵拉时间为非线性的正比量效关系;③牵拉对呼吸的影响只有牵拉达一定时间才表现出来。[Objective] To investigate the effects of graded intensity and time of visceral traction during operation on respiration and circulation. [Methods] Forty-two healthy Wistar rats were randomly divided into 7 groups. Group A received traction with 10g for 1 min. Group B received traction with 10g for 2 min. Group C received traction with 20g for 1 min. Group D received traction with 20g for 2 min. Group E received traction with 30g for 1 min. Group F received traction with 30g for 2 min. Group G (control group) was only operated but not dragged. Mean arterial pressure (MAP), heart rate (HR) and respiration rate (RR) were continuously monitored during experiment. [Results]There was significant difference in MAP between group D, E F and group A, B, C, G, and also between group A, B, C and group G. There was significant difference in RR between group B, D, F and group G, but there was no significant difference between group A, C, E and group G. [Conclusion](1)Traction on the jejunal vessels of rats can copy clinical traction reactions and establish a model of visceral traction intensity. (2)The increase of the intensity and time of traction can elevate the fluctuation range of MAP, but there is non-linear positive relationship between MAP and intensity and time of traction. (3)The effect of traction on respiration is displayed only when the time of traction is enough.
