支持性心理护理对脑卒中后抑郁症神经功能康复的影响

摘　　要：目的探讨支持性心理护理对卒中后抑郁患者神经功能康复的影响,并建立卒中后抑郁患者心理护理方案。方法将127例在我院神经内科治疗的脑卒中患者,随机分为治疗组和对照组,对照组予常规及抗抑郁药物治疗,治疗组除常规及抗抑郁药物治疗外另加支持性心理护理,分别在治疗前、治疗后按汉密顿抑郁量表(HAMD)和全国第四届脑血管病会议通过的神经功能缺损程度评分标准(CSS)进行评定。结果患者治疗前HAMD评分无明显差别(P>0.05),而治疗后2组HAMD评分则有明显的变化,治疗组优于对照组(P<0.001)。同样,CSS评分结果显示,治疗前治疗组与对照组之间的CSS评分无明显差别(P>0.05),治疗后2组CSS评分则有明显的变化,治疗组优于对照组(P<0.05)。结论支持性心理护理有利于脑卒中后抑郁症患者的心理康复和神经功能缺损的恢复。Objective To study supportive psychological care to patients with depression after stroke recovery of neurological function, and the establishment of post-stroke depression in patients with psychological care programmes.Methods 127 cases in the neurological department of our hospital of medical treatment of stroke patients were randomly divided into treatment and control groups,the control group used antidepressant drug treatment,psychological group in addition to antidepressant drug treatment,plus a supportive psychological care,in Before treatment,after treatment,respectively before treatment Hamilton Depression Scale（HAMD）,and the fourth cerebrovascular disease adopted at the score standards inform the assessment of the situation in patients with neurological rehabilitation.Results The patients treated before the Hamilton Depression Scale（HAMD）score no significant difference（P 〉0.05）,and after treatment Group 2 Hamilton Depression Scale（HAMD）score is a marked change,the treatment group distinctions In the control group（P〈0.05）.Similarly,CSS score showed that the treatment group and control group between the CSS score before treatment and no significant differences（p 〉0.05）, two groups of CSS score after treatment are obvious changes,the treatment group than the control group（P〈0.05）.Conclusion The support of psychological care to patients with post-stroke depression and psychological rehabilitation of neurological deficit recovery.
