内植法鼓膜修补术——自制鼓膜刮匙的应用及长期效果观察

摘　　要：目的探讨用自制刮匙刮除鼓膜穿孔缘鳞状上皮行内植法鼓膜修补术的临床效果。方法共对386例(398耳)用自制的鼓膜内层上皮刮匙刮除鼓膜穿孔缘残留鳞状上皮或掀起耳道深部皮肤,取颞肌筋膜行内植法鼓膜修补术。结果经2年以上随访,11耳再度穿孔,387耳穿孔修补成功。术后1至3个月纯音测听示听力改善率达97.2%。结论用自制刮匙刮除鼓膜穿孔缘残留鳞状上皮行内植法鼓膜修补术效果满意。Objective To report utility of a custom-made curette in myringoplasty procedures. Methods A custom-made curette designed by the authors was used in 386 cases（398 ears） of myringoplasty procedures to strip epithelial layer from perforation rims or to elevate skin flaps near the annulus fibrosus to prepare for repair of perforated tympanic membranes using the underlay technique with fascia tissue from the temporalis. Results At 2 year follow up, perforation recurred in only 11 ears. Hearing improvement was recorded in 97.2% of the ears when tested at 1 to 3 months following the procedure. Condlusion The curette facilitates recipient site preparation in myringoplasty procedures.
