摘  要:目的探讨股骨头骨骺软骨及生长板软骨生长、发育过程中的MRI特点及骨骺闭合的MRI表现。方法对40例正常人股骨头进行MRI扫描,按年龄分为4组,6个月以内5例,6个月~8岁13例,8~20岁12例,20~30岁10例。MRI扫描包括常规SET,WI、FSET1WI序列和sTIR—PDWI。结果MRI可以显示5种软骨结构,骨骺软骨在T1WI及T2WI均呈中等信号;生长板和二次骨化中心的生长板软骨T2WI呈稍高信号、T2WI呈高信号;临时钙化带及二次骨化中心临时钙化带在T1WI及T2WI均呈低信号。骨骺闭合后骺板STIR—PDWI像上由闭合前的高信号转变为低信号。结论MRI可以显示、区分生长发育期股骨头各种正常软骨结构;骺板在STIR—PDWI像上转变为低信号可以作为股骨头骨骺闭合的标准。Objective To study MRI findings of the normal femoral head epiphyseal and physeal cartilage with age,and femoral head physeal closure. Methods The femoral heads in 40 healthy subjects were examined with MR imaging by using SET1WI, FSE T2WI, STIR--PDWI. All cases were divided into 4 groups according to the age:〈6 months in 5,6 months- 8 years in 13,8- 20 years in 12 and 20~30 years in 10. Results MR images showed that the epiphyseal and physeal cartilage of femoral head had 5 kinds of different signal intensities on T1 WI and T2 WI. The epiphyseal cartilage was moderate signal intensity on T1 WI and T2 WI, but the low signal intensity band on T2WI could be found within the weight--bearing region after 1.5 years old. The growth plate cartilage and the growth plate cartilage of the secondary ossification center were slight hyperintense on T1WI and hyperintense on T2WI. The zone of provisional calcification and the zone of provisional calcification of the secondary ossification center were all hypointense on T1WI and T2WI. The physeal plates were low signal intensity on STIR-- PDWI after physeal closure. Conclusion Developmental cartilaginous changes in the femoral head are well seen at MR imaging. Epiphyseal plate in the STIR--PDWI changing into the low signal can be used as the stardard of closure of the femoral head epiphysis.
