摘  要:目的:比较Univent导管和双腔支气管导管(double-lumen endotracheal tube,DLT)在择期胸外科手术中用于单肺通气(one-lung ventilation,OLV)的临床效果。方法:择期胸外科手术患者72例,术中均需行OLV,随机分为两组:Univent组(U组)和DLT组(D组)。观察并记录:(1)完成插管和定位时间;(2)术中导管移位次数;(3)肺塌陷质量;(4)OLV期间气道峰压增加率;(5)气道损伤评分;(6)术后咽喉疼痛、声音嘶哑发生率。结果:两组间在完成插管和定位时间、术中导管移动次数及肺塌陷质量上无显著差异;D组单肺通气期间的气道峰压值增加率明显高于U组(P<0.05);U组气道损伤评分和术后咽喉疼痛、声音嘶哑发生率明显低于D组(P<0.05)。结论:Univent导管用于OLV时效果确切,与DLT相比可明显减少气道损伤,降低术后咽喉疼痛、声音嘶哑的发生率,有助于提高患者的麻醉满意度。Objective To compare the clinical efficacy of Univent tube and double-lumen endotracheal tube (DLT) used for one-lung ventilation in patients receiving elective thoracic surgery. Methods 72 patients needed one-lung ventilation during thoracic surgery were randomly assigned to receive one-lung ventilation with Univent tube (group U, n = 36) or with DLT (group D, n = 36). Parameters such as: time required for intubation and correct positioning, frequencies of malposition during operation, quality of lung collapse, rate of increased peak airway pressure ( a Ppeak) during one-lung ventilation, degree of tracheal and bronchial injuries, and incidence of sore throat and hoarseness after surgery were recorded. Results No statistical differences were found in the time required for intubation and positioning, frequencies of malposition during operation, and the quality of lung collapse between the two groups, aPpeak, degree of tracheal/ bronchial injuries, and incidence of sore throat and hoarseness after surgery were higher in group D than those in group U (P 〈 0.05). Conclusion Univent tube is effective for one-lung ventilation, it can reduce tracheal/bronchial injuries, decrease the incidence of sore throat and hoarseness after surgery, and can improve the patient's satisfaction of anesthesia.
