摘 要:目的:了解宁波市食品和公共场所从业人员乙肝携带者的基因型,为预防乙肝提供科学依据。方法:用ELISA法检测HBsAg及乙肝二对半,用手工法检测GPT,采用基因芯片对HBsAg及HBeAg阳性者进行基因分型。结果:15000名从业人员检出HBsAg阳性938名,阳性率为6.25%,发现HBsAg、HBeAb阳性(包括单项HBsAg阳性)、HBsAg、HBe-Ab、抗-HBc阳性(包括HBsAg、抗-HBc阳性)和HBsAg、HBeAg、抗-HBc阳性(包括HBsAg、HBeAg阳性)3种模式;不同性别和不同职业人员携带率差异无显著性;43份HBsAg、HBeAg、抗-HBc阳性携带者被分成B、C、BC、D4个基因型,分型率为93.5%,其中B型20份,占46.5%;C型21份,占48.8%;D型和BC混合型各1份,分别占2.3%。结论:从业人员仍有较高的乙肝携带率,其基因型以B、C型为主,与本地区病人的分型结果一致,C型的愈后不佳,提示我们应加强对从业人员的宣传力度,使从业人员充分认识到乙肝的危害,及时治疗,减缓病患的进展。在汉族人种检出D型比较少见,有待于进一步探讨和分析。Objective: To investigate the genotype of HBV carriers in employees for food and public work in Ningbo, which will do help to the prevention of HBV. Methods: ELISA was adopted to test HBsAg, HBsAb, HBeAg, HBeAb and HBcAb. GPT was detected by Manual method. HBV DNA was detected by gene chip in people whose HBsAg ang HBeAg is positive. Results:There were three modes among 938 of 15000 workers with positive HBsAg (the positive rate is 6. 25% ) :with positive HBsAg and HBeAb(including positive HBsAg alone) ;with positive HBsAg, HBeAb, and HBcAb (including positive HBsAg and HBcAb) ;with positive HBsAg, HBeAg and HBcAb( including positive HBsAg and HBeAg). There were no difference of the rate of HBsAg carrier among different gender and occupations. 43 cases with positive HBsAg, HBeAg and HBcAb were identified as four genetypes, the classification rate was 93.5%. Among those 43 cases ,20 were genotype B with the rate of 46. 5% ,21 were genotype C with the rate of 48. 8%, 1 was genotype D and B/C with the rate of 2. 3% respectively. Conclusion:There is still a higher rate of HBV carriers in practitioners with the predominant genotype of B and C, which is consistent with the results from the patients in Ningbo. Genotype C has Poor Prognosis. Datas above suggest that we should strengthen the publicity in practitioners and make them fully aware of the dangers of HBV. The disease should be treated in time to slow the progress of the patients'condition. Genotype D is rare in Han people, so we will study the phenomenon further.
