某院围手术期抗菌药物合理性应用调查分析

摘　　要：目的调查分析我院2008年下半年围手术期抗菌药物应用的情况,评估其用药的合理性。方法随机抽取我院2008年7至12月份的住院手术病历每月60份,总共360份,对围手术期抗菌药物应用进行合理性分析。结果我院抽查的360例手术患者的抗菌药物应用存在以下问题:①抗菌药物使用率过高,达96.11%;②药物选择起高点过高;③术后用药时间过长;④用药时机不规范;⑤无指征预防用药;⑥在药物的用法用量、药物更换以及联合用药指征的掌握、药物配伍等方面也存在不合理现象。结论我院围手术期抗菌药物的应用普遍存在不合理现象,应加强抗菌药物的使用管理,规范围手术期抗菌药物的使用原则,促进合理用药。Objective To investigate and analyze the usage of prophylactic antibiotics in perioperative period in our hospital at the second half year of 2008 and evaluate the rationalization of antibiotics use. Methods Random surveyed 360 medical records of surgeries who was on hospitalization from July 2008 to December 2008, 60 records in each month to analyze the rational use of Antibiotics in perioperative period. Results There are some problems about use of prophylactic antibiotics in perioperative period in our hospital 360 selected records as below： （1） Perioperative antimicrobials＇ utilization was too high, up to 96.11%. （2） The high level of antibiotics was chosen. （3） Inappropriate prolong of use medicine in postoperative. （4） Irregular timing of use medicine in perioperative period. （5） No administration according to the drug＇s pharmacodynamic feature. （6） There some unreasonable phenomenon of using Perioperarive Antimicrobials, such as the way and the quantity of use medicine, changing of medicine, inappropri- ate combined use of medicine and compatibility of medicine. Conclusion The irrational applications of prophylactic antibiotics in perioperative period are more serious. Hospital should strengthen the management of antibiotics; doctors should standardize the prophylactic use of antibiotic to ensure the drug application safety, effectively and economic.
