子宫动脉栓塞术治疗胎盘植入8例临床分析

摘　　要：目的探讨子宫动脉栓塞术治疗胎盘植入的临床疗效。方法对8例足月妊娠产后或中期妊娠引产后阴道出血量超过500mL的胎盘植入患者行双侧子宫动脉栓塞术治疗,以Seldinger技术经右侧股动脉穿刺插管,先行宫腔数字减影血管造影明确出血部位,再用直径1～3mm新鲜明胶海绵颗粒进行栓塞治疗。记录手术时间及栓塞后阴道的止血时间。结果8例患者均一次性止血成功,子宫动脉栓塞术的手术时间28～45min,平均(37±6)min;术后阴道止血时间5～10min,平均(7±3)min。结论应用子宫动脉栓塞术治疗胎盘植入,疗效确切、并发症少、止血迅速、安全可靠,且可以保留生育功能。Objective To evaluate the efficacy of uterine artery embolization（UAE） in placenta increata. Methods Eight cases of placenta increta with haemorrhage were treated by UAE. 6.0OF Cobra was placed into thedouble uterine arteries for treatment. The gelfoam particles mixed with antibiotic drugs vcas used to embolise the arteries. Results Among all 8 cases the bleeding was successfully controlled by one time of UAE. The duration of operation was 28 - 45min,mean（37 ± 6） min,hemostasis rime was 5 - 10min, mean（7 ± 3） min. After the treatment of UAE, the lactation and the menastrual periods of all patients were normal. Conclusion UAE is an effective saf etreat ment for placenta increta.
