氨氯地平对36例高血压病人左心室舒张功能的影响

摘　　要：目的：探讨氨氯地平对高血压患者左心室舒张功能的影响。方法：采用自身对照开放试验方法，观察氨氯地平对３６例高血压患者左心室舒张功能的影响，治疗前后作超声多普勒测量和二尖瓣血流速度变化分析，并进行对照分析。结果：３６例高血压治疗８周后，显效３０例，无效２例，总有效率为９４％。超声多普勒对照检查显示，总外周阻力（ＴＰＲ）、心肌重量（ＬＶｍａｓｓ）、室间隔舒张末期厚度（ＩＶＳｄ）下降，峰值充盈速度Ａ、峰值速度Ａ／峰值速度Ｅ比值下降，左心室舒张早期快速充盈速度（ＤＣ）上升，差异均具有显著性（Ｐ＜０．０５）。结论：氨氯地平在降压的同时，能有效改善左心室舒张功能。Objective:　To　study　the　effect　of　amlodipine　on　left　ventricular　diastolic　function　in　hypertensive　patients.　Methods:　A　self　control　open　clinical　trial　was　carried　out　to　inspect　the　effect　of　amlodipine　on　left　ventricular　diastolic　function　in　patients　with　hypertension.　Ultrasonic　scanning　and　mitral　blood　speed　analysis　were　performed　in　all　36　patients　and　results　were　compared　before　and　after　amlodipine　treatment.　Results:　After　8　weeks　of　treatment　with　amlodipine　(5　mg/d),　30　patines　achieved　excellent　therapeutic　effective,　2　patients　had　no　response.　The　total　effective　rate　was　94%　Ultrasonic　scanning　comparison　indicated　that　TPR,　LV mass,　IVSd　A　and　A/E　indexes　were　all　decreased,　while　DC　increased.　Significant　differences　among　these　indexes　existed　before　and　after　treatment　(P<005).　Conclusion:　Amlodipine,　could　not　only　decreases　blood　pressure,　but　could　also　improve　left　ventricular　diastolic　function　at　the　same　time.
