摘 要:目的探讨彩色多普勒超声对腹膜后脾移植术后移植脾组织的监测价值。方法应用彩色多普勒超声对9例腹膜后带血管蒂脾移植术后患者于术后1周、1月、6月进行动态监测,观察移植脾形态、大小、内部回声、血液供应及侧支循环建立情况。测量移植脾组织内动脉收缩期峰值流速(PSV)、舒张末期血流速度(EDV),阻力指数(R I)。结果移植脾组织显示率100%。17块(94.4%)移植脾组织术后1周显示为稍低回声,随时间推移,内部回声逐渐增强,术后6月,移植脾块接近正常脾脏组织回声,移植脾片内部回声不均匀,可见稍强回声区。术后1周移植脾组织内部探及少许血流信号,术后1月、6月,移植脾组织血供丰富,分布更加广泛,与腹膜后组织建立了侧支循环。移植脾组织内动脉PSV术后1月、6月明显高于术后1周(P<0.05),术后6月高于术后1月(P<0.05)。R I术后1月、6月低于术后1周(P<0.05),术后1月与6月相比差异无显著性(P>0.05);1块移植脾块术后1周中心呈等回声,周边为宽约0.5 cm的低回声带,内部未见血流信号,考虑坏死。结论彩色多普勒超声能有效监测移植脾的存活状态,可作为腹膜后脾移植术后移植脾的首选监测方法。Objective To investigate the value of color doppler ultrasonography in monitoring the retroperitoneal splenic autotransplantats. Methods Nine patients who underwent retroperitoneal splenic autotransplantation were examined by color doppler ultrasound to observe the shape, size,inner echogenicity,blood supply and collateral circulation of the transplanted spleen tissue one week, one month and six monthes postoperatively. Measure the peak systolic velocity (PSV) ,end- diastolic velocity (EDV), resistance index(RI) in the artery of the transplanted spleen tissue. Results The rate of the transplanted spleen appearce was 100%, seventeen pieces(94.4%) of the transplanted spleen tissue showed faint inner echogenicity. As time went by, the inner echogenicity increased. A week postoperatively, a little blood flow imaging was shown. Six months after the operation, there was sufficient blood supply and it was spread more widely. Extensive collateral circulationg was shown by color Doppler flow imaging. PSV in the artery of the transplanted spleen tissue one month and 6 months postoperatively were higher than that of one week postoperatively(P 〈0.05). The velocity of 6 months postoperatively was higher than that of one month postoperatively (P 〈0.05). RI of one month and 6 months postoperatively were lower than that of one week postoperatively(P 〈0.05) ,and there was no significance in RI of one month and 6 months postoperatively(P 〉0.05) While one week after the operation,one piece of the transplanted spleen tissue showed equal echogenicity,and there was a low chogenicity within a radius of 0.5 cm, no blood circulation was monitored, it was believed to have putrefied. Conclusion Color dopple ultrasound examination is an effective method to monitor the retroperitoneal splenic autotransplantats
