锁定钢板治疗肱骨近端粉碎性骨折疗效分析

摘　　要：目的观察肱骨近端锁定钢板治疗肱骨近端NeerⅢ、Ⅳ型骨折的临床疗效。方法2003年1月-2008年2月应用三叶草钢板和肱骨近端锁定钢板治疗肱骨近端骨折58例。观察组28例用肱骨近端锁定钢板内固定,NeerⅢ型20例,NeerⅣ型8例。对照组30例用三叶草钢板内固定,NeerⅢ型21例,NeerⅣ型9例。结果58例获随访8-36个月,平均15.6个月。按照Neer功能评定标准进行评分,观察组：优11例,良15例,可2例,差0例。对照组：优6例,良16例,可7例,差1例,采用SPSS11.0软件进行分析。观察组与对照组有显著差异（P〈0.05）。结论肱骨近端锁定钢板治疗肱骨近端NeerⅢ、Ⅳ型骨折是一种创新优异的方法。Objective To investigate the clinical outcome of the treatment with locking proximal humerus plate for Neer Ⅲ and Ⅳ proximal humeral fracture. Methods 58 cases of proximal humeral fracture from January 2003 to February 2008 were treated with the locking proximal humerus plate and the cloverleaf plate. Test group including 20 cases of Neer Ⅲ and 8 cases of Neer Ⅳ were treated with the locking proximal humerus plate. The other 30 eases including 21 cases of Neer Ⅲ and 9 cases of Neer Ⅳ as a control group were treated with the cloverleaf plate. Results All the patients were followed up for 8-36 mouths （ averaging 15.6 mouths）. According to Neer scoring system, in the test group, the therapeutic outcome was excellent in 11, good in 15 and so-so in 2 cases. The excellent and good rate was 92.9% ; in the control group ： the therapeutic outcome was excellent in 6, good in 16,so-so in 7 and poor in 1 cases. The excellent and good rate was 73.3%. The outcome between test group and control group was significant different （ P 〈 0.05 ）. Conclusion The treatment of locking proximal humeral plate for Neer Ⅲ and Ⅳ proximal humeral fracture is a good innovational method.
