摘 要:目的:研究X射线CT医疗照射所致上海市公众的剂量负担。方法:采用面上普查的方式,分头部、体部调查上海市各医院CT检查年频率,并调查X射线CT检查中成年人头部、胸部、腹部检查部位的标准扫描条件,结合CT剂量指数的测量结果和应用CT剂量评价软件,估算上海市X射线CT医疗照射所致公众的剂量负担。结果:上海市目前有在用医用X射线CT机151台,2007年CT检查年频率为128.16人次/千人口。在头部、胸部、腹部等常规扫描中,一次检查所致受检者有效剂量分别为0.93mSv、4.6mSv和4.9mSv。2007年上海市X射线CT医疗照射所致全市公众集体剂最是4868人·Sv,人均年有效剂量0.35 3mSv。结论:X射线CT医疗照射所致公众剂量负担呈增长趋势,频率水平和单次检查有效剂量的增长是导致公众剂量负担增加的主要原因。Purpose: To study the dose burden of the public in Shanghai induced by medical exposure from X - ray computed tomography. Methods: The general condition of medical X- ray CT scanner application in Shanghai and the annual frequency distribution of medical exposure from X - ray CT examination in 101 hospitals were investigated on head and body examinations respectively. The head and body dose index of 151 medical X- ray CT scanners were measured. Meanwhile, the standard protocols relating to the head, chest and abdomen routine scans on adults were collected by questionnaires. And based on the annual frequency results in Shanghai, the software of CT dose assessment was applied to evaluate the collective dose of the public in Shanghai induced by medical exposure from X - ray computed tomography. Results:At present, there were a total of 151 medical X - ray CT scanners in 101 hospitals to carry out the medical X - ray CT procedures in Shanghai district. In 2007, the annual frequency distribution of CT examination was 128.16 person - time per 1000 population. The effective dose of patient from each examination was 0.93 mSv, 4.6 mSv and 4.9 mSv in the head, chest and abdomen routine scan, respectively. The collective effective dose of the public in Shanghai district from X - ray computed tomography medical exposure was 4868 man · Sv, and the average annual individual effective dose was 0. 353 mSv in 2007. Conclusion: The collective dose of the public from X - ray computed tomography medical exposure has an increasing trend, and the main reason is the increase of frequency level of CT medical exposure and effective dose from one CT scan.
