夹层动脉瘤42例治疗分析

摘　　要：目的:对8年来所进行的主动脉夹层动脉瘤患者的治疗状况进行总结分析。方法:42例患者分别采用Bentall手术、David手术、单纯升主动脉置换术、Bentall手术+全弓置换+硬象鼻支架植入术、David手术+全弓置换+硬象鼻支架植入术、单纯升主动脉置换+全弓置换+硬象鼻支架植入术、覆膜血管支架腔内隔绝术和全胸腹降主动脉置换术,并进行3个月～3年随访。结果:完成Debakey Ⅱ型13例,Debakey Ⅰ型16例,DebakeyⅢa型5例,DebakeyⅢb型8例,无围手术期死亡。全部患者随访3个月～3年,均生存。结论:主动脉夹层动脉瘤手术复杂多样,应根据主动脉根部病变的情况选择升主动脉置换术式。Objective： Summary and analyse the results of aortic dissecting aneurysm surgical treatment during the past 8 years. Methods： The surgical treatment in 42 patients included Bentall＇s procedures, David procedures, ascending aorta replacement procedures, Bental procedures combined with arch replacement and Stented elephant trunk implantation, David procedures combined with arch replacement and stented elephant trunk implantation, ascending aorta replacement combined with arch replacement and stented elephant trunk implantation, endovascular stent-graft exelusion procedures, total descending aorta replacement procedures. All the patients had been followed up for 3 months to 3 years. Results： There were 13 Debakey Ⅱ patients, 16 Debakey Ⅰ patients, 5 DebakeyⅢa patients, 8 Debakey Ⅲb patients underwent surgical treatment and no perioperative death. All the patients were alive during 3 months to 3 years follow-up period. Conclusion： The procedures of aortic dissecting aneurysm were complicated. Which procedure should be performed depended on the pathological process of aortic root.
