摘  要:目的评估Siemens64层螺旋CT扫描机Lung Care软件的计算机辅助检测(CAD)系统检出肺结节的能力,并评价其对放射科医师检出肺结节准确性的影响。方法纳入53例检出肺结节的胸部MDCT患者,由不同水平三位放射科医师(R1、R2、R3)分别于不使用和使用CAD系统的情况下独立阅片检测肺结节。应用受试者工作特征曲线(ROC曲线)判断不使用和使用CAD时放射科医师表现的差异。结果根据参考标准共检出161个肺结节。对直径<4 mm、≥4 mm及所有结节,阅片者独立阅片的ROC曲线下面积均值(Az值)分别为0.54、0.64、0.61;而使用CAD二次阅片时,Az值分别升至0.87、0.81、0.84。对各组结节,阅片者使用CAD组的Az值均显著大于独立阅片组。结论无论阅片者有经验与否,本研究所用CAD系统均可帮助放射科医师提高其对肺结节的检出率,特别是对直径小于4 mm的小结节。Objective To evaluate the performance of computer-aided detection (CAD) of Lung Care software in detecting pulmonary nodules on Siemens Sensation 64-slice computed tomography, and to assess the effect on the nodule detection when using CAD. Methods Totally 53 thoracic multislice computed tomography (MDCT) examinations with pulmonary nodules were interpreted independently by three radiologists (R1, R2, and R3) with different levels of experience. Nodules were detected by the radiologists first without and then with CAD. The performances of the radiologists without and with CAD were compared, using receiver operating characteristic curves analysis. Results They detected 161 nodules according to the reference standard. For nodules 〈4 mm, nodules≥4 mm and all nodules, the average area under the free-response receiver operating characteristic curve (Az average) for the radiologists without using CAD was 0.54, 0.64 and 0.61, respectively. With the aid of CAD, the Az average increased to 0.87, 0.81 and 0.84. There was a significant difference in Az value between the readers group with and without CAD among the above three nodule groups. Conclusion The CAD system used in our study can assist radiologists in detecting pulmonary nodules for both experienced or less experienced readers, especially in the detection of small nodules (〈4mm).
