药疹病人EB病毒和人疱疹病毒6型感染检测

摘　　要：目的探讨EB病毒(EBV)和人疱疹病毒6型(HHV6)感染在药疹发生中的作用。方法采用PCR-Southern blotting检测药疹病人和正常对照组外周血单个核细胞中EBV DNA;采用巢式聚合酶链反应检测HHV6 DNA,并对HHV6阳性标本进行酶切分型;采用ELISA法检测EBV VCA-IgM;采用RT-PCR-Southernblotting检测EBV即刻早期基因BZLF1 mRNA。结果药疹病人EBV DNA阳性率(77.42%,24/31)明显高于正常对照组(χ2=77.84,P<0.01),但HHV6阳性率(32.26%,10/31)与正常对照组比较差异无显著性(χ2=3.00,P>0.05)。10例HHV6阳性药疹病人全部为HHV6A亚型,未检测到HHV6B亚型;而正常对照组73例HHV6阳性标本中有13例为HHV6A亚型。药疹病人EBV VCA-IgM阳性率(19.34%,6/31)与正常对照组比较差异具有显著意义(χ2=4.61,P<0.05)。10例HHV6阳性药疹病人中有9例检出EBV DNA,5例BZLF1 mRNA阳性病人中有4例检出HHV6 DNA,药疹病人BZLF1 mRNA检出率与HHV6有明显的相关性(r=0.45,P<0.05)。结论药疹病人存在EBV的活动性感染,且EBV和HHV6可能有相互激活作用。Objective To study the role of Epstein-Barr virus （EBV） and Human Herpesvirus 6 infection in drug-eruption patients. Methods EBV DNA was tested in drup-eruption patients and normal control by PCR-Southern blotting, and HHV6 DNA by nested-polymerase chain reation, the positive samples were then genotyped, the EBV VCA-IgM was tested by ELISA. The expression of BZLF1 mRNA was tested by RT-PCR-Southern in EBV positive samples. Results The positive rate of EBV DNA （24/31,77.42%） in the patients was higher than the normal control （χ2= 77.84,P〈0.01）, but the difference of HHV6 was not significant （P〉0.05）. In 10 HHV6-positive patients, all were HHV6A subtype, no HHV6B subtype was found; but in 73 positive eases of the normal control, HHV6A subtype was noted in 13. The positive rate of EBV VCA-IgM in the patients was 19.34% （6/31）, the difference was significant as compared with the normal control （χ2=4.61,P〈0.05）. In 10 HHV6 positive patients, nine were detected to have EBV DNA; in five with BZLF1 mRNA positive, four were noted with HHV6 DNA, the detection rate of BZLF1 mRNA was dramatically correlated with HHV6 （r=0.46,P〈0.05）. Conclusion There is an active infection of EBV in patients with drug rash, a reciprocal activation may exist EBV and HHV6.
