摘  要:目的:研究血管紧张素转换酶(ACE)(I/D)、醛固酮合成酶(CYP11B2)-344T/C、α-内收蛋白(α-adducin)460G/T不同基因多态性与高血压患者及正常人脉压(PP)的关系。方法:按PP<40 mmHg、40~59 mmHg、≥60 mmHg,高血压组(1081例)和血压正常组(604例)分别被分为3组,计数资料用卡方检验,计量资料用t检验或方差检验。结果:高血压组和血压正常组在二项逐步Logistic回归中,调整了其他混杂因素后,两组ACE-DD+CYP11B2-CC/TC+α-adducin-TT型和年龄均进入最后的方程;两组ACE-DD+CYP11B2-CC/TC+α-adducin-TT型与高脉压(≥60 mmHg)分别显著相关(OR值分别为3.318和4.556,P分别为0.005和0.044)。结论:在高血压患者和正常人中,同时携带有ACE基因DD型、CYP11B2基因TC/CC型和α-adducin基因TT型者脉压增大的风险要高于其他基因型组合,是脉压增大的独立危险因素。Objective: To investigate the association among ACE, CYP11B2 and α-adducin gene polymorphisms with pulse pressure (PP) in primary hypertensive and normotensive subjects. Methods: The primary hypertensive subjects (1081 cases) and normotensive subjects (604 cases) were divided respectively into three groups: PP〈40mmHg, 40-59mmHg and ≥60mmHg. Binary stepwise Logistic regression was used to analyze the relationship between gene poly-morphisms and PP, which being considered confounding factors like age, sex, etc. Results: Binary Logistic stepwise regression results demonstrated that ACE--DD+CYP11B2-CC/TC+α-adducin-TT genotype and age entered the model after adjusting confounding factors. ACE-DD+CYP11B2-CC/TC+αadducin-TT genotype was significantly associated with high pulse pressure (≥60mmHg) in both primary hypertensive (OR= 3. 318, P=0. 005) and normotensive groups (OR=4. 556, P=0. 044). Conclusion: The risk of high PP increased in both primary hy=pertensive and normotensive subjects whom carried ACE-DD, CYP11B2-TC/CC and α-adducin TT genotypes at the same time, and this combination of genotype is the independent risk factor of high PP.
