流式细胞术与定量PCR法检测HLA-B27的比对

摘　　要：目的评价实时荧光定量PCR法和流式细胞术两种方法检测人类白细胞抗原HLA-B27对强直性脊柱炎的临床诊断价值。方法将疑似强直性脊柱炎患者199例全血分别采用实时荧光定量PCR和流式细胞仪法检测HLA-B27,以流式细胞术HLA-B27阳性作为判断标准,确定实时荧光定量PCR法检测HLA-B27的敏感度、特异度、阳性预测值及阴性预测值。将流式细胞术HLA-B27阳性的样本44例,按照实时荧光定量PCR检测结果将其分为疑似组、阴性组及阳性组,确定有无表达抗原表达强度的差异。结果91.5%样本分型结果相同,差异不具有统计学意义(P<0.05)。实时荧光定量PCR检测HLA-B27的敏感度为95.1%,特异度达到96.8%,阳性预测值为88.6%,阴性预测值为98.7%。实时荧光定量PCR检测疑似组、阴性组及阳性组,无显著表达抗原表达强度的差异。结论实时荧光定量PCR与流式细胞术检测HLA-B27具有相似敏感度和特异度,是一种较为快速、简单、有效的检测方法。Objective To evaluate the diagnostic values of Real-time fluorescence quantitative PCR methods and flow cytometry in detecting HLA-B27.Methods Real-time fluorescence quantitative PCR,flow cytometry were used to detect HLA-B27 in 199 samples. Use the positive result of flow cytometry as-standard, to investigate the sensitivity, specificity, predictive value of a positive test, predictive value of a negative test of Real-time fluorescence quantitative PCR. The people that were HLA-B27 positive by flow cytometry were divided into ankylosing spondylitis group, healthy control group and unidentified group according to the result of Real-time fluorescence quantitative PCR and evaluated the difference of Expression of HLA-B27 antigen among the groups. Results The sensitivity, specificity, predictive value of a positive test, predictive value of a negative test of Real-time fluorescence quantitative PCR were 95.1%, 96.8 %, 88.6 %, 98.7 % respectively. There was no significant difference among the three groups（ P 〈 0.05）. Conclusion The Real-time fluorescence quantitative PCR method is an accurate and effective detection method for the diagnosis of AS.
