中药糖脉康改善2型糖尿病胰岛素抵抗的临床观察

摘　　要：[目的]探讨益气养阴活血中药(糖脉康)改善气阴两虚血瘀型2型糖尿病患者胰岛素抵抗的临床疗效。[方法]将138例2型糖尿病伴胰岛素抵抗患者随机分为对照组和治疗组。两组基础治疗均为口服降糖西药并进行饮食控制,治疗组在此基础上加用中药糖脉康。3个月为1个疗程。主要观察治疗前后空腹血糖(FBG)、餐后2h血糖(2h BG)、空腹血胰岛素(Fins)、糖化血红蛋白(HbA1c)、血脂、凝血功能等指标的变化,并计算胰岛素抵抗指数(HOMA-IR)。[结果]两组患者治疗3个月后,FBG、2h BG、Fins、HbAlc、HOMA-IR及血脂、凝血指标均有不同程度的明显改善,但以治疗组改善更明显。[结论]益气养阴活血中药在防治和改善2型糖尿病患者的胰岛素抵抗及血脂紊乱、血液高凝状态方面具有一定的疗效,与降糖西药联用具有相互协同和增效作用。[Objective] A Research on the effect of Chinese medicine Tangmaikang（TMK） on improving Insulin Resistance（IR） in type 2 diabetes mellitus was performed. [Methods] The therapeutic effect of integrative use of TMK having effect of supplementing qi, nourish- ing yin and activating blood circulation on the improvement IR of type 2 diabetes was observed. One hundred and thirty-eight IR patients were randomly divided into 2 groups （control group and treated group, 68 patients in control group and 70 in treated group）. They were treated respectively with western medicine and dietary control in control group, while in addition to the above treatment, the treated group were also given the Chinese medicine TMK. The observation period in two group patients was 3 months. Fasting blood glucose （FBG）, blood glucose 2 hours after meal （2hBG）, fasting insulin（Fins）, glycosylated hemoglobin （HbAlc）, blood fat, blood coagulation were ob- served before and after treatment, and the index of IR was calculated. [Results] After 3 months of treatment, FBG, 2hBG, Fins, HbAlc, HOMA-IR, blood fat, blood coagulation had obvious improvement in varied level. [Conclusion] The use of Chinese medicine TMK can prevent and improve insulin resistance of type 2 diabetic patients, improve blood fat disorder and serum hypercoagulation state.
