摘  要:目的评价实时荧光定量逆转录聚合酶链反应(FQ-RT-PCR)和免疫组织化学法(IHC)测定食管癌组织中血管内皮生长因子-C(VEGF-C)和血管内皮生长因子受体-3(VEGFR-3)基因表达在诊断食管癌淋巴结转移中的意义。方法分别采用实时FQ-RT-PCR和IHC检测食管癌组织中VEGF-C/VEGFR-3 mRNA和VEGF-C/VEGFR-3蛋白。结果有淋巴结转移组食管癌组织VEGF-C/VEGFR-3 mRNA和蛋白的表达均显著高于无淋巴结转移组;实时FQ-RT-PCR检测VEGF-C mRNA诊断食管癌淋巴结转移的敏感性、特异性以及与病理学诊断的符合率与IHC检测VEGF-C蛋白差异无统计学意义;实时FQ-RT-PCR检测食管癌组织VEGFR-3 mRNA诊断食管癌淋巴结转移的敏感性、特异性以及与病理学诊断的符合率均明显高于IHC(P<0.05)。结论实时FQ-RT-PCR检测VEGF-C/VEGFR-3 mRNA可较好地反映食管癌淋巴结转移状况。Objective To evaluate the significance of real-time fluorescent quantitative reverse transcription polymerase chain reaction (FQ-RT-PCR) and immunohistoehemistry (IHC) detecting the vascular endothelial growth factor-C (VEGF-C) and vascular endothelial growth factor receptor-3 (VEGFR-3) gene expression in diagnosing lymphatic metastasis of esophageal carcinoma. Methods The real-time FQ-RT-PCR and IHC were used to detecting VEGF-C and VEGFR-3 genes and their protein expressions in esophageal carcinoma. Results VEGF-C and VEGFR-3 mRNA and their protein expressions in carcinoma tissue from patients with lymphatic metastasis of esophageal carcinoma were significantly higher than that in carcinoma tissue from patients without lymphatic metastasis of esophageal carcinoma. The sensitivity, specificity, conformity with pathological diagnosis of real-time FQ-RT-PCR detecting VEGF- C mRNA in carcinoma tissue on diagnosing lymphatic metastasis of esophageal carcinoma were not significantly higher than IHC detecting protein. The sensitivity, specificity, conformity with pathological diagnosis of real-time FQ-RT-PCR detecting VEGFR-3 mRNA in carcinoma tissue on diagnosing lymphatic metastasis of esophageal carcinoma were significantly higher than IHC detecting protein ( P 〈 0.05 ). Conclusions The real-time FQ-RT-PCR is a better method in diagnosing lymphatic metastasis of esophageal carcinoma.
