脑脊液负流量置换联合鞘内注药治疗结核性脑膜炎临床分析

摘　　要：目的探讨脑脊液负流量置换联合鞘内注药治疗结核性脑膜炎的临床疗效。方法80例结脑患者随机分为治疗组和对照组,所有患者均采用3HRZE(S)/9HRE方案抗结核治疗,治疗组患者同时采用脑脊液负流量置换联合鞘内注药,然后进行对比。结果治疗组临床症状改善明显快于对照组,脑脊液蛋白、白细胞数和脑脊液压力下降的速度等方面比较差异有统计学意义。治疗8周时2组总疗效比较有显著性差异。未发现与脑脊液负流量置换联合鞘内注药相关的并发症。结论脑脊液负流量置换联合鞘内注药治疗结核性脑膜炎安全有效,值得临床推广使用。Objective To explore the curative effect of the treatment of negative flow of substituting cerebrospiral fluid and intrathecal injection on the tuberculosis cephalomeningitis. Methods Eighty cases of tuberculosis cephalomeningitis patients were randomly divided into treatment group（42 cases） and control group（38 cases）. Both groups received the 3HRZE（S）/ 9HRE treatment, while treatment group was also treated by negative flow of substituting cerebrospiral fluid and intrathecal injection, then comparision was made between the two groups. Results Improvement of clinical symptoms in treatment group was faster than that in control group. There was significant difference between the two groups in the reduction rate of protein,leukocyte and the pressure of cerebrospiral fluid. By the end of 8th week, there was significant difference between the two groups in the total curative effect. No complications and obvious adverse reactions were observed in the process of substituting cerebrospiral fluid and intrathecal injection. Conclusion The treatment of negative flow of substituting cerebrospiral fluid and intrathecal injection on the tuberculosis cephalomeningitis is safe and more effective than simple chemotherapy in the treatment of tuberculosis cephalomeningitis. It is worthwhile that the clinical application should be approached extensively.
