腹部术后宽肠理气汤灌肠对Ghrelin表达的影响

摘　　要：目的:观察宽肠理气汤术后灌肠对胃肠功能及对Ghrelin表达影响,并探讨其作用机制。方法:40例患者分为胃肠手术组、肝胆手术组,每组随机分为应用宽肠理气汤术后灌肠的中药组和常规治疗的对照组,分别于术前及术后1、3、5d观测Ghrelin、EGG主频(DF)、正常慢波百分比(N%)变化以及排气排便时间。结果:各组术后DF和N%均较术前下降,但中药组较对照组恢复快并且第5d时更接近正常值。术后排气时间与排便时间,肝胆组比胃肠组恢复快。Ghrelin与胃肠功能恢复正相关。结论:宽肠理气汤术后灌肠可促进胃肠功能的恢复,并与调节内源性Ghrelin的水平关系密切。Objective To investigate the influence of Kuan Chang Li Qi（宽肠理气） decoction on gastrointestinal and Ghrelin secretion after abdominal operation, and to approach the mechanism of the action of Kuan Chang Li Qi decoction. Methods Forty cases were selected and divided into two groups on the basis of the procedure： hepatobiliary surgery and gastrointestinal surgery, 20 in each group. Each group was divided into two subgroups randomly, treated with Kuan Chang Li Qi decoction enema on postoperative 1 to 3 days and with routine treatment （control group） respectively. The level of Ghelin, EGG dominant frequency （DF） and normal rhythm （N%） were measured pre and post - operatively on days 1,3,5. APACHE Ⅱ system was used to evaluate the patient＇ s condition. Results Gastrointestinal function recovered earlier in Chinese herbal group than in control significantly, though DF and N% was lower than the one before operation. The level of Ghrelin after surgery increased significantly （ P 〈 0.05）, decreased post - operation day 3. The level of serum Ghrelin was positively correlated with the gastrointestinal function recovered. Conclusion The study showed that Kuan Chang Li Qi decoction can promote gastrointestinal function recovery, and was positively correlated with the regulation of Ghrelin secretion.
