血清白介素18高敏C反应蛋白水平与冠心病的关系

摘　　要：目的:探讨冠心病患者血清白介素18、高敏C反应蛋白水平与冠心病临床表现及冠状动脉病变程度的关系。方法:63例冠心病患者和对照组16例,分为ACS组、SAP组和对照组。所有入选对象均进行冠状动脉造影检查,冠心病患者分为单支病变组、双支病变组、多支病变组。检测所有对象血清白介素18及高敏C反应蛋白水平。结果:随着冠心病病情危险程度的升高,白介素18、高敏C反应蛋白水平逐渐升高,各组间差别具有统计学意义(P<0.01,P<0.05),两者水平变化经Pearson相关性检验呈正的直线相关关系,(r=0.425,P<0.05)。随着冠脉病变程度的加重,两者水平变化差别无统计学意义(P>0.05)。结论:白介素18、高敏C反应蛋白水平可以作为冠心病危险性的预测因子。两者血清水平与冠状动脉病变程度无明显相关,与冠状动脉造影联合使用可以更好的评估冠心病患者的预后。Objective： To investigate the relationship between IL - 18, hs - CRP level and clinical manifestation of coronary heart disease and to investigate the relationship between them and severity of coronary artery lesions at the same time. Methods： 63 patients of coronary heart diseases and 16 healthy control persons were enrolled. They were divided into three groups： ACS group, SAP group and the control group. All cases were performed coronary angiography. The patients were divided into three groups as fol- lows：single vessel disease group, two vessels disease group, multiple vessels disease group. The concentration of IL- 18 and hs- CRP in serum was determined. Results ： As compared with that of the control, the patients with ACS or SAP had significantly higher IL- 18 and hs- CRP concentration（ P 〈0.01, P 〈 0.05）.ACS compared with SAP, The levels of IL - 18,hs - CRP all raised significantly（ P 〈 0.05）. Correlation analysis indicated that levels of IL- 18 were positively correlated with hs- CRP in all cases（ r =0.425, P 〈0.05）.The levels of IL - 18,hs - CRP were no difference existed in different vessels disease groups （ P 〉 0.05）. Con- clusion ： IL - 18,hs - CRP all take part in the course of CHD. IL - 18,hs - CRP are related to the seriousness of CHD, but they are not associated with severity of coronary artery lesion. They can well help evaluating severity and the outcome of coronary heart disease with coronary angiography.
