极外侧型腰椎间盘突出症诊治体会

摘　　要：目的：分析极外侧型腰椎间盘突出症的临床特点及手术治疗方式的选择。方法：回顾性分析经手术证实的5例极外侧型腰椎间盘突出症的临床资料，均采用椎板外横突间途径行髓核摘除术治疗。结果：本组5例，均经手术治疗，随访9个～23个月，平均15．6个月，效果优良。结论：重视临床症状、体征与影像学资料的紧密结合，以提高诊断的准确性。CT检查清楚显示位于椎管外的椎间盘突出物，对于术式的选择有重要价值。Objective： To analyse clinical characteristics and choice of surgical approaches in extreme lateral prolapse of inter vertebral disc. Methods： In a retrospective study, 5 cases with extreme lateral prolapse of inter vertebral disc were treated surgically by the outer lamina Transverse Process resection of nucleus pulposus. Results ： The 5 cases with surgical approaches had satisfactory results at follow- up from 9 months to 23 months（mean of 15.6 months）. Conclusion： Clinical symptoms, signs, imaging data should been closely combined to achieve accurate diagnosis. CT which clearly scans intervertebral disc protuberance out of vertebral canal has an important effect on choosing surgical approaches.
