摘 要:目的探讨不同解剖结构肾下盏的尿流动力学变化。方法采用非线性有限元分析系统分别模拟不同肾下盏肾孟夹角、肾下盏长度、盏颈宽度条件下成人上尿路的不间断传输活动,并对集合系统内的尿流情况进行流体力学分析,对比肾下盏开口处压力、流速最大值(P、V)。结果肾下盏肾孟夹角60°、肾下盏长度为50mm或盏颈宽度为3.6mm时,恒流实验发现肾盂出口最大流速、压力均变小;恒压实验发现肾孟出口最大流速变小,最大压力变大。结论肾下盏内的尿液流动是尿液的势能和上尿路的泵功能(具体表现为肾盂和肾下盏内的压力、尿液流速)共同作用的结果,解剖结构改变对肾下盏的尿液传输有影响。Objective To evaluate the urodynamics of the low calyx which had the different anatomical features. Methods To creat the two dimensional digital modal of the collecting system being noncoaptive transportation and analyse the flow dynamic with the Automatic Dynamic Incremental Nonlinear Analysis System(ADINA). The maximum velocity(V) and the maximum pressure(P) in the pelvic and low calyx junction were test. Results When the infundibulopelvic angle was 60 degrees, the length of the inferior calyx was 50 mm or the width of the inferior calyx was 3.6 mm. It was found that the velocity and the pressure were less in the constant velocity perfusion trial and the velocity were also less in the constant pressure perfusion trial, but the pressure were more in the constant pressure perfusion. Conclusions The urine flow in the low calyx is the result of the potential energy and the upper urethral. It should be effected by the anatomical feature.
