桂哌齐特治疗后循环TIA的疗效及对血液流变学和血流动力学的影响

摘　　要：目的:观察桂哌齐特治疗后循环短暂性脑缺血发作(TIA)的疗效及对血液流变学和血流动力学的影响。方法:将76例后循环短暂缺血发作患者随机分为治疗组和对照组各38例,分别给予马来酸桂哌齐特注射液和血塞通注射液静滴治疗。观察2组临床疗效,治疗组在治疗前后进行血液流变学和血流动力学测定。结果:经Ridit分析,2组疗效差异有显著性(P<0.05);治疗组的显效率为84.2%,显著高于对照组的52.6%(P<0.01)。治疗组在治疗后血液流变学各项指标均较治疗前有显著改善(P<0.05和P<0.01)。经颅多普勒(TCD)观察到治疗组在治疗后峰值流速(Vp)、平均流速(Vm)、血管搏动指数(PI)均有显著改善(P<0.01)。结论:桂哌齐特治疗后循环短暂性脑缺血发作安全、有效,能明显改善患者的血液流变学和血流动力学指标。OBJECTIVE To observe the therapeutic effect of cinepazide in the treatment of transcient ischemia attack（TIA） of posterior circulation and its influence on hemorrheology and hemodynamics. METHODS 76 cases with TIA of posterior circulation were divided randomly into therapy group and control group, receiving treatment of intravenous cinepazide maleate injection and Xuesaitong injection respectively. The clinical efficacy of two groups was observed of two groups, hemorrheology and hemodynamics of therapy group were examined before and after treatment. RESULTS There was significant difference of clinical efficacy between two groups（P^0. 05）. The improving rate of therapy group was significantly higher than that of control group（84. 2% vs 52. 6%,P〈0. 01）. Comparison with those before treatment, the indices of hemorheology in therapy group improved obviously after treatment（P〈0. 05 or P〈0. 01）. Values of systole peak flow velocity （Vo）, the mean flow velocity （Vm） and the pulsating index （PI） by transcranial doppler（TCD） in therapy group improved significantly after treatment （P〈0. 01）. CONCLUSION Cinepazide is an safe and effective drug for the the treatment of TIA of posterior circulation and can effectively improve the indices of hemorrheology and hemodynamics.
