艾司洛尔对烧伤创面肾上腺素液湿敷止血的心血管反应平抑效果观察

摘　　要：目的:观察艾司洛尔对大面积烧伤切削痂术中应用肾上腺素液湿敷创面止血的心血管反应平抑效果。方法:30例大面积烧伤切痂手术患者随机分为实验组与对照组,每组15例,实验组患者在创面应用肾上腺素液湿敷时给予艾司洛尔,对照组则给予生理盐水,记录2组患者在肾上腺素盐水湿敷前1min(T0),湿敷后1min(T1),5min(T2),10min(T3),15min(T4),30mim(T5)不同时点的平均动脉压(MBP),心率(HR)。结果:对照组T2,T3,T4的MBP及HR较T0时点有显著性增加(P<0.01),实验组T0,T2,T3,T4的MBP及HR各时点差异均无显著性(P>0.05),实验组与对照组间在T2,T3,T4时点的MBP,HR有显著性增加(P<0.01)。结论:艾司洛尔对烧伤创面肾上腺素液湿敷止血的心血管反应有很好的平抑作用。OBJECTIVE To evaluate inhibitory effect of esmolol to cardiovascular response induced by adrenalin wet compression to stop bleeding in the excision of eschar in patients with large area burn wound. METHODS Thirty patients were include in this study and divided into two groups （n = 15）. Esmolol was injected intravenously when adrenalin wet compression was applied to burn wound in experimental group, while saline was injected in control group. Mean blood pressure and heart reat were documented lmin before application of adrenalin （To）, and 1 min （T1）, 5 mn（T2 ）, 10 min（T3 ） 15 min（T4 ）, 30 mim （T5） after application of adrenalin. RESULTS MBP and HR increased significantly in T2, T3, T4 when compared with T0 （P〈0. 01）in control group. No significant difference were seen in experimental group in MBP？ between T0,T2, T3, T4, and in HR between all time spoiffts. MBP and HR increased significantly in control group in T2, T3,T4 when compared with experimental group. CONCLUSION Esmolol inhibits cardiovascular response induced by adrenal wet copression to hemostasis in burn wound.
