摘  要:目的探讨乙型肝炎病毒(HBV)基因突变与宫内传播的关系。方法采用聚合酶链反应(PCR)方法从HBsAg阳性孕妇及其新生儿血清中扩增得到包括HBV前C/C区和前S/S区基因的2.5kb基因片断或前S/S区1.2kb基因片断,克隆、测序,分析测序结果,比较母婴HBV基因序列的差异。结果4对发生宫内感染的母婴HBV序列共得到43株,都属于HBVC基因亚型、adr血清亚型。HBV前S/S区序列中,新生儿序列的差异性低于母体(P<0.05)。4对母婴间序列的平均相似率分别为99.47%、99.39%、99.37%和99.46%;母婴序列具有高度一致性;每对母婴序列间的进化距离都比较小,核苷酸差异率分别为0.4、0.8、1.2和0.3。母婴序列间均存在1个或数个核苷酸位点的差异,母体内的优势株和新生儿体内的优势株不完全相同。结论母体内的优势株或弱势株都可以通过宫内传播感染新生儿,可能与其基因突变有密切关系。提示HBV的宫内传播存在基因选择性。Objective To investigate the relationship between HBV intrauterine transmission and gene mutations. Methods 2.5 kb or 1.2 kb HBV DNA fragments were amplified by PCR from sera of the new neonates and their HBsAg-positive mothers.After purified, the PCR products were cloned into pUC19 vector and the HBV sequences were determined and analyzed, then the differences in sequences of each pair of mother and neonate were compared. Results All 43 isolates of HBV from 4 pairs of infected mothers and neonates were C gene and adr serum subtype. In HBV pre-S/ S domain, the divergence of sequences of neonates was lower than that of their mothers ( P 〈 0.05). The average similarity rates of nucleotide sequences in each pair of mother and neonate were 99.47 %, 99.39 %, 99.37 %, and 99.46 % respectively, so there was a remarkable consistency. The phylogenetic trees were constructed from pre-S/S domain of nucleotide sequences of each pair mother and neonate. The evolutionary distance was 0.4, 0.8, 1.2 and 0.3 respectively. There were some mutation positions in nucleotide sequences between mother and neonate. The predominant strains in mothers and neonates were not completely consistent. Conclusion Both HBV predominant strains and minority strains in mothers may infect their neonates through intrauterine transmission, which may relate to HBV gene mutation, suggesting that intrauterine infection may have gene selectivity.
